<p>Position Summary: Provides a range of psychological assessment, diagnostic, therapeutic, and/or case management services for individuals, children, couples, groups, and/or families, as appropriate to position. Will participate in case consultation and present cases at clinical meetings.</p> <p>Essential Functions & Responsibilities:</p> <ul> <li> <p>Advanced Clinical diagnostic assessment/documentation skills</p> </li><li> <p>Including but not limited to psychosocial assessments, progress notes, telephone consultation, brief assessments, patient education</p> </li><li> <p>Ability to assess suicide and homicide risk potential</p> </li><li> <p>Planning for improving health services by interpreting social factors pertinent to development of the program</p> </li><li> <p>The ability to work within a Multidisciplinary Team Model</p> </li><li> <p>Individual, Group and Family therapy skills</p> </li><li> <p>Strong ability to problem solve</p> </li><li> <p>Community Collaboration skills</p> </li><li> <p>Skills to assess needs and programs accordingly</p> </li><li> <p>To practice in the scope of their training, qualifications and experience</p> </li><li> <p>Adherence to their professional code of ethics</p> </li><li> <p>Strong knowledge of and connection to community resources</p> </li><li> <p>Additional duties as requested</p> </li><li> <p>School Based Health Center (If applicable)</p> </li><li> <p>An understanding of the workings of school systems</p> </li><li> <p>Prevention and intervention skills and strategies appropriate for school settings</p> </li><li> <p>The ability to independently (with support) develop a caseload while working within the school system</p> </li></ul> <p>Additional General Requirements: Professional positive attitude, understanding of customer service principals, trustworthiness and excellent interpersonal skills.</p> <p>Job Qualifications/Requirements: To perform this job successfully, an individual must be able to perform his/her duties satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties.</p> <p>Education: LCSW position requires a Master's degree from an accredited program in Social Work.</p> <p>Experience: Must have previous experience as an LCSW in a professional clinical setting, maintaining an EHR, while providing therapeutic services. Group and family treatment preferred but not required.</p> <p>Language Skills: Must speak, write and read English proficiently. Spanish preferred but not required.</p> <p>Licensure/Certification Required: LCSW position requires a Master's degree from an accredited program in Social Work, a current license from the Connecticut State Board of Registration and passed a national or state-certified licensing exam and is CPR certified.</p>
