For more information, visit www.oakstreethealth.com.</p> <p>Program Overview</p> <p>@HOME delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care to the population attributed to Oak Street Health's Managed Service Organization (MSO). Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of our patients throughout the healthcare delivery system.</p> <p>Role Description</p> <p>The Clinical Instructor has the unique opportunity to continue to evolve existing onboarding and clinical performance initiatives for the At-Home Program. Current initiatives focus on education of providers to support patient care from Day 1 and throughout a provider's tenure. The Clinical Instructor serves as a clinical coach to deliver, enhance and develop onboarding lectures, large scale training sessions, simulation programs and 1:1 support initiatives to both new and veteran providers. The Clinical Instructor will support in executing organizational strategies, feedback review and support future iterations of our learning and onboarding platforms.</p> <p>Candidate Responsibilities</p> <ul> <li> <p>Onboarding. The Clinical Instructor serves to support onboarding and clinical development needs of providers, behavioral health specialists and care teams. This includes,evolving and developing educational content, delivering lectures up to 4 days per week, and implementing standardized, case-based, simulation training to test competencies in assessment, management, technology and workflows.</p> </li><li> <p>Content Development. Partnering with operational and clinical leaders , the Clinical Instructor builds and oversees an educational program tailored to developing high performing, multidisciplinary mobile teams with deep understanding of delivering value-based care. Content will focus on high value chronic conditions, hospitalization prevention, and end of life care including the technologies, coding/documentation requirements, and workflows that support those areas of focus. Content development will involve shadowing and real-world testing of workflows and clinical skills, and will serve both new onboarding teams as well as existing veteran teams.</p> </li><li> <p>Clinical development. The Clinical Instructor collaborates with operational and clinical leaders to determine strategies to improve clinical performance and outcomes that support our operational goals. Collaborations will include deep engagement with operational and training teams to effectively design clinical content and delivery platforms for our rapidly expanding in-home workforce.</p> </li></ul> <p>What We're Looking For</p> <p>This is an exciting opportunity to be an operational leader in a value based care organization that is changing the way healthcare is delivered.
