The Clinical Nurse works with other team members in a team-based model of care, coordinating, planning, assessing, and evaluating care services to ensure safe and effective patient care. Additional activities include assisting with staff development/education specific to Adolescent Medicine, quality improvement activities, and performing other duties to facilitate coordination of care for patients.</p> <p>The Clinical Nurse will work with highly skilled team members to provide high value patient care in an organization focused on patient safety, patient and parent satisfaction and positive patient outcomes. As a team, our objective is to establish and maintain effective processes and collaborative working relationships.</p> <p>The University of Utah offers a comprehensive benefits package including:</p> <ul> <li>Excellent health care coverage at affordable rates </li><li>14.2% retirement contributions that vest immediately </li><li>Generous paid leave time </li><li>11 paid Holidays per year </li><li>50% tuition reduction for employee, spouse, and dependent children </li><li>Flex spending accounts </li><li>Free transit on most UTA services </li><li>Employee discounts on a variety of products and services including cell phones & plans, entertainment, health and fitness, restaurants, retail, and travel </li><li>Professional development opportunities </li></ul> <p>Additional benefits information is available at https://benefits.utah.edu/</p> <p>Responsibilities</p> <p>Responsibilities</p> <ul> <li>Assist families, providers and Pediatrics Clinical Enterprise in navigating legislative changes and facilitating access to care with other in state or out of state providers (when applicable). </li><li>Coordination of care with other in state or out of state providers (when applicable). </li><li>Experience caring for adolescents, including transgender, obesity, eating disorders, moving from a pediatric to adult provider, and other adolescent specific conditions. </li><li>Assess patient and family needs for clinical services; communicate directly and indirectly with healthcare providers to appropriately coordinate all aspects of patient care. </li><li>·Maintain and create resources for patients and families. </li><li>Implement processes to improve patient care and clinic experience - patient tracking and follow-up reminders in EMR (Electronic Medical Record). </li><li>Assist patients in understanding how to access insurance coverage and benefits - often including calling to advocate on behalf of patients and families. </li><li>Reach out to community resources to advocate for patients at their schools and in extracurricular environments. </li><li>Participate in outreach events and educational presentations in the community. </li><li>Coordinate medication refills, letters of medical necessity, DMV forms, school care plans and medication prior authorizations as needed. </li><li>Coordinates quality improvement activities. </li><li>Refer and follow-up on referrals made to psychiatric and medical providers. </li><li>Coordinate admissions to higher levels of care for patients and families. </li><li>Facilitate urgent visits for patients outside of provider templates (involves coordination with scheduling, medical assistants, and providers). </li><li>Coordinate visits with multiple providers in monthly, multidisciplinary specialty clinics. </li><li>Schedule patient visits by acuity and provider request - often coordinating with outside treatment teams to coordinate care for patients. </li><li>Prepare and send patient briefs to providers prior to clinic. </li><li>Manage clinic flow, ensure medications are stocked and ready for patients on day of clinic and that paperwork and consents are available to patients prior to visits. </li><li>Perform other related duties as assigned. </li></ul> <p>Work Environment and Level of Frequency that may be required:</p> <p>Nearly Continuously: Office environment.</p> <p>Often: Infectious disease, oils (there is air or skin exposure to oils or other cutting fluids), hazards (includes a variety of physical conditions, such as proximity to moving mechanical parts, moving vehicles, electrical current, working on scaffolding and high places, or exposure to chemicals).</p> <p>Physical Requirements and Level of Frequency that may be required</p> <p>Nearly Continuously: Hearing, listening, talking.</p> <p>Often: Repetitive hand motion (such as typing), walking, standing.</p> <p>Seldom: Bending, reaching overhead.</p> <p>Minimum Qualifications</p> <p>Required</p> <p>Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.</p> <p>Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.</p> <p>Preferred</p> <p>Some positions may require additional certifications such as Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), or Neonatal Resuscitation Program (NRP) within one year of hire.</p> <p>This position is patient-sensitive and must fulfill all associated requirements.
