<p>04125 LPN Rehab STRHS-Lawrenceburg Job Description</p> <p>Job Summary</p> <p>As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.</p> <p>Reports to: Director of Rehab</p> <p>FLSA: Non-exempt</p> <p>Essential Functions</p> <p>Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.</p> <p>Performs plan of care interventions, including specimen collection and wound care. May administermedication. Documents patient care given.</p> <p>Assists RN with development of plan of care.</p> <p>Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.</p> <p>Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.</p> <p>Works as an advocate for the physical and emotional well-being of the patient.</p> <p>Nonessential Functions</p> <ul> <li>Perform other duties as assigned.• Attends and participates in staff meetings, in-service, projects and committees as assigned.• Adheres to and supports policies and procedures of the STRHS.• Works scheduled shifts including overtime, when necessary.• Accepts all call schedule as directed.• Maintains a neat and professional appearance in compliance to the existing dress code. </li></ul> <p>Organizational Expectations</p> <ul> <li>Compliance to the STRHS - Lawrenceburg Standards of Performance is required.• Compliance to Dress Code Standards• Should this job description be filled by a PRN employee, they will be subject to the requirements of that position. </li></ul> <p>Job Requirements</p> <p>Minimum EducationCurrent and active Licensed Practical Nurse in the State of Tennessee.</p> <p>Required SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
