<p>Overview</p> <p>Alliance Cancer Specialists is seeking a highly motivated and detail-oriented Clinical Pharmacist to join our outpatientoncology team. This role is integral to ensuring safe, effective, and cost-efficient medication use for oncology patients, with a strong emphasis on formulary management, therapeutic drug substitutions,and oversight of oral chemotherapy dispensing.</p> <p>KEY RESPONSIBILITIES</p> <p>Formulary Management</p> <ul> <li>Maintain and update the oncology formulary in collaboration with the Pharmacy & Therapeutics </li></ul> <p>Committee.</p> <ul> <li>Evaluate new oncology medications and therapies for inclusion based on clinical efficacy, safety, </li></ul> <p>and cost-effectiveness.</p> <ul> <li>Monitor utilization trends and recommend changes to optimize formulary adherence. </li></ul> <p>Drug Substitution & Therapeutic Interchange</p> <ul> <li>Assess and implement therapeutic substitutions in alignment with institutional protocols and </li></ul> <p>evidence-based guidelines.</p> <ul> <li>Collaborate with oncologists and care teams to ensure appropriate substitutions that maintain </li></ul> <p>therapeutic intent.</p> <ul> <li>Educate staff and providers on substitution policies and rationale. </li></ul> <p>Oral Medication Dispensing & Management</p> <ul> <li>Oversee the dispensing of oral oncology agents, ensuring accuracy, safety, and compliance with </li></ul> <p>regulatory standards.</p> <ul> <li>Provide patient counseling on oral chemotherapy regimens, including adherence strategies and </li></ul> <p>side effect management.</p> <ul> <li>Monitor refill adherence and intervene as needed to support continuity of care. </li></ul> <p>Clinical Support & Collaboration</p> <ul> <li>Participate in multidisciplinary rounds and tumor boards as needed. </li><li>Provide drug information and clinical decision support to providers and nursing staff. </li><li>Assist in developing and updating treatment protocols and order sets. </li></ul> <p>Quality & Compliance</p> <ul> <li>Ensure compliance with USP standards, state and federal regulations, and institutional policies. </li><li>Participate in quality improvement initiatives related to oncology pharmacy services. </li><li>Document interventions and outcomes in the electronic health record. </li></ul> <p>QUALIFICATIONS</p> <ul> <li>Doctor of Pharmacy (PharmD) degree required. </li><li>Completion of PGY1 residency; PGY2 in Oncology Pharmacy preferred. </li><li>Current pharmacist license in Pennsylvania. </li><li>Board Certification in Oncology Pharmacy (BCOP) preferred. </li><li>Minimum 2 years of experience in oncology pharmacy practice, preferably in an outpatient </li></ul> <p>setting.</p> <p>Skills & Competencies</p> <ul> <li>Strong clinical knowledge of oncology therapeutics. </li><li>Excellent communication and collaboration skills. </li><li>Proficiency in EMR systems and pharmacy dispensing software. </li><li>Ability to manage multiple priorities in a fast-paced environment </li></ul> <p>Physical & Work Environment Requirements:</p> <ul> <li>Ability to stand and walk for extended periods and lift up to 40 lbs </li><li>Manual dexterity and visual acuity required for patient care tasks </li><li>Exposure to communicable diseases, toxic substances, and medical preparations common to oncology clinics </li><li>In-person interaction with patients, physicians, and clinical staff is essential </li></ul> <p>Why Join Alliance Cancer Specialists?</p> <p>At Alliance, you'll be part of a mission-driven team that's making a real difference in the lives of patients.
