The Certified Nursing Assistant is a paraprofessional member of the Interdisciplinary</p> <p>Team who performs various services for a patient and family as necessary to meet the patient's personal needs.</p> <p>T h e C N A is responsible for observing, reporting, and documenting care performed.</p> <p>Essential Job Functions</p> <ul> <li>Responsible for performing personal care needs after being given a patient's assignment and attending </li></ul> <p>to his/her requests promptly. If unable to perform a certain task, report to the RN immediately.</p> <ul> <li>Responsible for meeting the safety needs of patients and using equipment safely and properly </li><li>Responsible for giving direct personal care, including complete or partial baths in the tub or </li></ul> <p>shower as ordered, back rubs, oral hygiene, hair care, shaving (using an electric razor only), and</p> <p>changing bed linen as often as assigned.</p> <ul> <li>Responsible for assisting in the dressing and undressing of patients as assigned. </li><li>Responsible for providing proper care and observation of the patient's skin to prevent breakdown of </li></ul> <p>tissue over bony prominence.</p> <ul> <li>Responsible for reporting on the patient's condition and significant changes to the </li></ul> <p>assigned RN</p> <ul> <li>Responsible for being aware of and reporting on the status changes </li></ul> <p>of the caregiver or other individuals living with the patient, and interpersonal issues that may come up</p> <ul> <li>Assist in ambulation and exercise as instructed by the RN or therapist while adhering to the </li></ul> <p>transfer/assist transfer policy of the agency.</p> <ul> <li>Assistance with self-administered medications as allowed by state regulations ('reminders.") </li><li>Responsible for providing incontinent care as needed. Offering and assisting with bedpans and </li></ul> <p>urinals. Providing assistance as assigned with light laundry needs.</p> <ul> <li>Responsible for keeping the patient's living area clean and orderly, including bed linens and cloths as </li></ul> <p>needed while being respectful of the patient and family/caregiver environment and personal needs as time allows</p> <ul> <li>Responsible for adhering to the organization's documentation and care procedures and standards </li></ul> <p>of personal and professional conduct.</p> <ul> <li>Other office /administrative duties as assigned </li></ul> <p>Qualifications</p> <p>(Minimum qualifications will be considered required unless specifically stated otherwise)</p> <p>Experience:</p> <ul> <li>Minimum of one (I) to two (2) year's experience in health care preferred. </li><li>Experience in acute care in an institutional setting preferred. </li><li>Experience in homecare preferred. </li></ul> <p>Education and Training:</p> <p>(Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination</p> <p>of these. May be expressed in terms of formal educational equivalents):</p> <ul> <li>High school diploma or equivalent. </li><li>Satisfactory completion of a training program approved by M.D.P.H. for HCA-II and </li></ul> <p>other regulators.</p> <ul> <li>Training equivalent to an approved certification program (such as C.N.A. or Student Nurse), </li></ul> <p>applicants must be oriented to home care.</p> <p>License, Certification & Registration:</p> <ul> <li>Possesses and maintains current CPR Certification. </li><li>Current Massachusetts licensure: Certified Nursing Assistant </li><li>Valid driver's license and automobile insurance </li></ul> <p>Other Requirements:</p> <ul> <li>Demonstrates good verbal and written communication, and organization skills. </li><li>Strong observation skills and problem solving skills. </li><li>Ability to read and follow written instructions and document care given. </li><li>Ability to provide a calm and empathetic demeanor in patient's homes. </li></ul> <p>Job Limitations:</p> <p>The CNA will not function in any manner viewed as the practice of nursing</p> <p>according to the Nurse Practice Act Specifically, the CNA will not</p> <p>administer medications, take physician's orders or perform procedures requiring training,</p> <p>knowledge and skill of a licensed nurse, such as sterile technique.</p> <p>Working Conditions</p> <ul> <li>Uniform/ Dress Requirements (Beyond those requirements outlined within Policy No. </li></ul> <p>C:3-015.1):None</p> <p>Working Conditions:</p> <ul> <li>May be exposed to extremes of heat and cold in all weather conditions. Contact with patients under a wide variety </li></ul> <p>of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying</p> <p>and unpredictable situations.
