<p>POSITION SUMMARY:</p> <p>The Community Health Worker (CHW) role works as part of the Women's Health Department team to coordinate patient care and provide case management services. The CHW links patients to services and services as well as provides follow-up and encourages positive health behavior changes.</p> <p>Monday - Friday - 40 hours</p> <p>Four(4) 10 hour shift or Five(5) 8 hour shifts.</p> <p>Bilingual Required (Portuguese, Spanish, and or Cape Verdean Creole).</p> <p>ESSENTIAL FUNCTIONS:</p> <p>Reasonable accommodations may be made to enable individuals with disabilities to perform the following essential functions.</p> <ol> <li> <p>Retains patients in care through outreach and case management while supporting patients with chronic disease self-management.</p> </li><li> <p>Promote patient treatment adherence through assessing patient readiness to make changes, assisting patients in making changes to daily routines through the use of motivational interviewing, and assisting patients in developing strategies to address barriers.</p> </li><li> <p>Conduct needs assessments, including assessing barriers and assets, transportation, language literacy and cultural preferences.</p> </li><li> <p>Identifying community-based resources to refer and connect patients with appropriately. Engage patients through the process- application, follow-up, and coordination of services. These resources will address patient needs related to social determinants of health (e.g. food, shelter, utilities, etc.).</p> </li><li> <p>Performs HIV and STI Counseling & Testing for patients.</p> </li><li> <p>Document within the electronic medical record and fill out other required paperwork.</p> </li><li> <p>Data collection and entry into CAREWARE or other data base.</p> </li><li> <p>Attend and participate in supervision, staff meetings, and trainings to promote professional development.</p> </li><li> <p>Represent the Health Center at community outreach events</p> </li></ol> <p>Qualifications</p> <p>REQUIRED KNOWLEDGE & SKILLS:</p> <ol> <li> <p>Bilingual required (e.g. Portuguese, Spanish, or Cape Verdean Creole)</p> </li><li> <p>Communicate effectively in a culturally competent manner</p> </li><li> <p>Organization and time management skills</p> </li><li> <p>Ability to work with diverse patient population.</p> </li><li> <p>Sensitivity to unique needs of patients with HIV and other related health conditions.</p> </li></ol> <p>REQUIRED EDUCATION & OTHER CREDENTIALS:</p> <ul> <li>Education: High school graduate or GED </li><li>Experience: Experience with HIV/AIDS, Viral Hepatitis, infectious diseases, or sexual health desirable. </li><li>Certification/Licensure: MA Certified Community Health Worker preferred </li><li>Software/Hardware: Microsoft Office required.
