Apply today and be part of a mission-driven team providing compassionate end-of-life care.</p> <p>Summary of Responsibilities:</p> <p>The Hospice Volunteer Coordinator is responsible for developing, implementing, and supervising the volunteer program in accordance with federal and state regulations. This role ensures the effective integration of volunteers into the hospice interdisciplinary team and maintains compliance with CMS Conditions of Participation and Pennsylvania Department of Health guidelines.</p> <p>The content of this job description is intended to describe the general nature and level of work expectations performed by incumbents in this classification. All activities are completed in accord with the mission, vision, and values of the Company, following all policies, procedures, Equal Employment Opportunity laws, patient confidentiality, and other Federal, State and Local laws and regulations, as appropriate.</p> <p>Essential Functions/Duties:</p> <ul> <li>Program Development & Management </li><li>Plan, coordinate, and manage all aspects of the hospice volunteer program. </li><li>Develop policies and procedures for volunteer services consistent with federal and state regulations. </li><li>Ensure that volunteers contribute at least 5% of total patient care hours, as required by CMS. </li><li>Recruitment, Training & Supervision </li><li>Recruit, screen, and interview potential volunteers. </li><li>Provide initial orientation and ongoing education based on hospice-specific training standards. </li><li>Maintain documentation of volunteer training, supervision, evaluations, and service hours. </li><li>Ensure volunteers receive appropriate support and are matched with tasks suitable to their skills and preferences. </li><li>Interdisciplinary Collaboration </li><li>Participate in interdisciplinary team meetings to ensure appropriate volunteer assignment and involvement in patient care. </li><li>Coordinate volunteer services with clinical staff to support patients, caregivers, and administrative needs. </li><li>Regulatory Compliance & Documentation </li><li>Maintain compliance with requirements for volunteer services in hospice programs that participate in Medicare and Pennsylvania hospice regulations. </li><li>Keep accurate records of volunteer activity and ensure data is included in required reports to CMS and state regulators. </li><li>Prepare annual evaluation reports of the volunteer program and recommend improvements. </li><li>Community Outreach </li><li>Promote the hospice volunteer program through presentations, partnerships, and community events. </li><li>Serve as a liaison between the organization and the community to raise awareness about hospice and volunteer opportunities. </li></ul> <p>Other Duties:</p> <ul> <li>As assigned by Management </li><li>Essential functions are not intended to be an exhaustive list of all position duties required and performed by the incumbent in the role. Nothing in this job description restricts. </li><li>management's right to assign or reassign duties and responsibilities to this job at any time. </li><li>Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and Inservice classes. </li><li>Fulfills the obligation of requested and/or accepted case assignments. </li><li>Actively participates in quality assessment performance improvement teams and activities </li></ul> <p>Education and Experience Requirements:</p> <ul> <li>High school diploma or equivalent required. </li><li>Associate's or Bachelor's degree in human services, social work, health care administration, or related field preferred. </li><li>Minimum of one (1) year experience in a health care setting; hospice or palliative care experience preferred. </li><li>Prior experience coordinating or managing volunteers is strongly preferred. </li><li>Valid Pennsylvania Driver's License and acceptable Motor Vehicle Record Check </li><li>BLS/ CPR certification </li></ul> <p>Required Skills/Abilities:</p> <ul> <li>Strong organizational and communication skills. </li><li>Ability to work independently and collaboratively as part of a team. </li><li>Comfortable working with individuals in emotionally sensitive and end-of-life situations. </li><li>Competency with EMR systems and Microsoft Office (Word, Excel, Outlook). </li><li>Ability to travel locally to visit patients and volunteers as needed. </li></ul> <p>Supervisory Responsibilities:</p> <ul> <li>Supervises ancillary personnel and delegates responsibilities when required. </li></ul> <p>Position Type/Expected Hours of Work:</p> <ul> <li>The position is Monday through Friday part-time. Days and hours worked to be determined based on organizational needs. Hospice business office hours are from 8:00am to 4:30pm. </li></ul>
