Together, we can make a significant difference in the lives of children and contribute to a brighter and healthier future for all.</p> <p>Summary:</p> <p>Provides case management services to an assigned patient population, including but not limited to, referrals, follow-up phone calls and triage. Provides support that requires clinical expertise in various systems with focus on helping the family negotiate the complexities involved with a diagnosis. Each BHCC provides case management support to patients. The BHCC is responsible for providing social service referrals for patients and families, including housing, transportation, medical and mental health services. They ensure the preparation and maintenance of required documentation per accreditation standards and CHST policy. The BHCC works closely with a team of Behavioral Health Care Managers and consulting psychiatrists to ensure patients are adhering to their plans of care by communicating with patient families, identifying barriers to care, and helping to ameliorate those barriers when possible. The BHCC is part of a multi-disciplinary treatment team and will work to build collaborative relationships with UTSW providers to ensure the best treatment outcomes for our patients.</p> <p>Responsibilities:</p> <ul> <li>Attends and participates in team meetings with team lead as well as monthly departmental discussions. </li><li>Collaborate with inpatient treatment team to prevent gaps in care and provide discharge planning support. </li><li>Assists patient families navigate the behavioral health delivery system based on their clinical need and financial/insurance coverage. </li><li>Facilitate parent/child referrals to address social service needs and supportive therapy. </li><li>Coordinate and schedule: psychiatric consultations, ongoing care with licensed providers, LMHAs, and support groups. </li><li>Support licensed behavioral health clinicians by outreaching families under their supervision. </li><li>Assists in identifying patient's insurance coverage and facilitate access to health information, community resources, and social services. </li><li>Participate in rounds/treatment/consultation meetings as well as monitor the patient's progress toward the desired outcome. </li><li>Collaborate with providers to establish treatment milestones to prepare patient and family for transition to lower level of care. </li><li>Oversee the flow of treatment information across providers and within the care team in order to maximize patients physical and emotional well-being. </li><li>Facilitate referrals to the appropriate areas to expedite care, treatment and services. Establish lines of communication with ancillary services. </li><li>Participate in the development, implementation, evaluation and ongoing revision of the plan of care, clinical pathways and initiatives to improve quality, continuity and cost effectiveness. </li><li>Assesses patient functioning and escalates to the treating Behavioral Health Care Manager or consulting psychiatrist when clinically appropriate. </li><li>Work collaboratively with other departments and services to define and study areas of inefficiency and participate in process improvement projects. </li><li>Be involved in the development of strategies and plans to maximize the most appropriate use of services in assigned areas. </li><li>Submit to and pass external background check processes as required per vendor or funder requirements. </li></ul> <p>How You'll Be Successful:</p> <p>WORK EXPERIENCE</p> <ul> <li>At least 3 years previous experience in social services or healthcare or related experience required </li></ul> <p>EDUCATION</p> <ul> <li>Four-year Bachelor's degree or equivalent experience required </li><li>Graduate or professional work or advanced degree; or equivalent experience Social Work, Psychology, Marriage and Family Therapy or related discipline preferred </li></ul> <p>JOB PROFILE</p> <ul> <li>Requires experience in own professional discipline to appropriately apply own knowledge; broadens own professional expertise and acquires higher level skills </li><li>Understands key business drivers; uses this understanding to accomplish own work </li><li>No supervisory responsibilities but may provide informal guidance to new team members </li><li>Solves problems in straightforward situations; analyzes possible solutions using professional/technical experience, judgment and precedents </li><li>Impacts quality of own work and the work of others on the team; works within guidelines and policies </li><li>Receives a moderate level of guidance and direction </li><li>Explains complex information to others in straightforward situations </li></ul> <p>A Place Where You Belong</p> <p>We put our people first.
