<p>Job Title: Psychiatric Nurse Practitioner</p> <p>Department: Behavioral Health</p> <p>Reports To: Medical Director</p> <p>FLSA Status: Exempt</p> <p>Salary: $100,000 - $125,000</p> <p>POSITION SUMMARY</p> <p>The Psychiatric Nurse Practitioner position will play a key role in the fast-growing Behavioral Health Department of our multi-discipline primary care community health center. This position will be part of our evolving health services as we move to an integrated care model. As that happens, in addition to providing direct care to patients, this position will also help support the primary care providers and care managers in treating patients with behavioral health problems. The right candidate will thrive in a fast-paced environment where the first concern is providing outstanding patient care.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES</p> <ul> <li>Perform initial and ongoing clinical evaluations </li><li>Responsible for medication management and treatment planning </li><li>Perform crisis intervention by telephone for patients during non-office hours as assigned </li><li>Consult and advise the clinical staff and Clinical Director </li><li>Manage care coordination with all other appropriate care providers </li><li>Enter data directly into the Center's electronic medical records system </li><li>Work with children/adolescents and adults </li><li>Performs and/or assists in the administration of routine diagnostic/laboratory studies, clinical and therapeutic procedures </li><li>Develops presumptive nursing diagnoses and implement treatment plans based on advanced </li><li>nursing assessment/problem identification consistent with established practice guidelines. </li><li>Documents assessments of patients' health and illness status. </li><li>Evaluates patient responses to treatment plans and redirects patient care based on findings. </li><li>Assesses patient clinical/health education needs. Instructs/counsels' patients, in one-on-one and small group settings, regarding health matters (e.g. diets, disease therapy, normal growth and development, etc.). </li><li>Refers patients to clinical/health education classes, as appropriate for more in-depth services. </li><li>Provides telephone care to assigned patients, as necessary. </li><li>Obtains assigned patient vital signs and provide documentation. </li><li>Provides onsite patient care and outreach for our School Based Health Clinic and/or Mobile Health Unit. </li><li>Acts as a resource person to other staff members. </li></ul> <p>QUALIFICATIONS</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.</p> <ul> <li>Master's Degree: Psychiatric Mental Health Nurse Practitioner </li><li>Illinois Licensure </li><li>3+ years of clinical experience demonstrable experience in diagnosis </li><li>Team player </li><li>Excellent interpersonal skills </li></ul> <p>EDUCATION AND EXPERIENCE</p> <ul> <li>Completion of an accredited Master's Degree program for Psychiatric Nurse Practitioners. </li><li>Experience based on current needs of the practice </li></ul> <p>CERTIFICATES AND LICENSES</p> <ul> <li>Current Illinois RN </li><li>CPR Certification </li></ul> <p>Language Skills</p> <p>Ability to read, comprehend and communicate detailed instructions. Ability to prepare correspondence and memos. Ability to effectively present information in one-on-one and small group situations to patients, physicians and other employees of the organization.</p> <p>Reasoning Ability</p> <p>Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
