<p>Job Purpose</p> <p>We are seeking a highly organized and detail-oriented Scheduling Coordinator to join our team in the Program of All-Inclusive Care for the Elderly (PACE). The Scheduling Coordinator will play a crucial role in managing and coordinating appointments for participants of the PACE program, ensuring they receive comprehensive and timely care services. This position requires strong communication skills, multitasking ability, and a commitment to providing excellent customer service to elderly individuals.</p> <p>Duties and Responsibilities</p> <ul> <li>Schedule and coordinate medical, therapeutic, and other health-related appointments for PACE participants in collaboration with healthcare providers and the interdisciplinary care team. </li><li>Maintain accurate and up-to-date participant schedules, ensuring all appointments are scheduled in accordance with participants' needs and care plans. </li><li>Communicate effectively with participants, families, and healthcare providers regarding appointment availability, changes, and cancellations. </li><li>Monitor and track appointments to ensure all services are provided timely and efficiently, following up as necessary. </li><li>Assist in managing transportation arrangements for participants, ensuring they have access to transportation for their scheduled appointments. </li><li>Handle the rescheduling of appointments when needed, ensuring minimal disruption to participant care. </li><li>Maintain detailed records of appointments, cancellations, and rescheduling within the program's scheduling system. </li><li>Collaborate with interdisciplinary teams to ensure that care plans align with scheduled appointments and services. </li><li>Ensure that all appointment scheduling complies with program policies and regulatory requirements. </li><li>Assist with reporting and data entry as required, providing updates on appointment status and trends. </li><li>Act as a liaison between participants, families, providers, and internal teams to resolve any scheduling-related concerns or issues. </li><li>Other duties as assigned. </li></ul> <p>Qualifications</p> <p>Education:</p> <ul> <li>High school diploma or equivalent required; Associate's degree or higher preferred. </li><li>Previous experience in healthcare scheduling, administrative support, or customer service, particularly in the elderly or senior care field, is preferred. </li><li>Strong organizational skills with the ability to manage multiple tasks and prioritize effectively. </li><li>Excellent communication skills, both written and verbal, with the ability to interact professionally with participants, families, and healthcare providers. </li><li>Detail-oriented with the ability to maintain accurate records and handle confidential information. </li><li>Proficiency with scheduling software and office tools such as Microsoft Office (Word, Excel, Outlook). </li><li>Knowledge of the PACE program and its services is a plus. </li><li>Ability to work independently as well as in a team-oriented environment. </li><li>Compassionate and empathetic toward elderly individuals and their families. </li></ul> <p>Desired Personal Attributes:</p> <ul> <li>Dependable, flexible and resourceful. </li><li>Enjoys working with disabled and older adults. </li><li>Effective interpersonal skills to work with members, caregivers and staff. </li><li>Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds. </li><li>Ability to be self-directed and able to work independently. </li><li>Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds. </li></ul> <p>Other:</p> <ul> <li>Must be able to work required schedule. </li><li>Requires physical strength to perform essential functions of the job. </li><li>Requires use of personal vehicle. </li><li>Requires valid driver's license. </li><li>Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage: $100,000/$300,000 personal liability and $100,000 property damage. </li><li>May require use of personal cell phone for business purposes (may be eligible for stipend) </li></ul> <p>Physical Demands</p> <ul> <li>Must be able to remain in a stationary position 50% of the time. </li><li>Ability to occasionally move about inside the office to access file cabinets, office machinery, etc. </li><li>Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer. </li><li>Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc. </li><li>The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role. </li></ul> <p>Compliance with the Americans with Disabilities Act (ADA)</p> <p>Our center is dedicated to fostering an environment that is inclusive and accessible, welcoming everyone with open arms.
