<p>Description:</p> <p>A First Name Basis is one of the fastest-growing in-home care providers with 40+ offices across four states. We're reimagining what it means to serve seniors and individuals with disabilities-by building strong caregiver careers, implementing smart clinical and scheduling systems, and ensuring compliance and care quality are never compromised.</p> <p>We are seeking a detail-oriented and enthusiastic LPN to join our team! This role will be a mix of in office and home visits.</p> <p>Schedule: Monday - Friday, 8:30am-5pm</p> <p>Position Summary:</p> <p>The LPN is responsible for assessing the medical needs of potential and current clients, ensuring that services provided are appropriate and meet their healthcare requirements.</p> <p>Responsibilities:</p> <ul> <li>Develop and update comprehensive care plans. </li><li>Provide staff training, ensure accurate documentation, and support medication management. </li><li>Conduct in-home health assessments, administer IM and SC injections, and address medical concerns. </li><li>Supervise caregivers for proper medication administration, vital sign documentation, and protocol compliance. </li><li>Manage client medications, coordinate refills, and liaise with pharmacists. </li><li>Lead caregiver training and evaluate competency in essential medical tasks. </li><li>Maintain accurate health records and electronic documentation. </li><li>Communicate effectively with families, caregivers, and team members regarding client needs and updates. </li><li>Work under RN supervision while operating independently in the field. </li></ul> <p>Benefits:</p> <ul> <li>Competitive pay </li><li>Medical, dental, vision benefits </li><li>Cell phone reimbursement </li><li>401(k) with employer match </li><li>PTO </li><li>Sick time </li><li>Paid company holidays </li></ul> <p>Requirements:</p> <ul> <li>Licensure: Valid State LPN License. </li><li>Compact license preferred. </li><li>Education: High school diploma or GED required. </li><li>Experience: Minimum 1 year in home care, healthcare, or senior-related industry (preferred). </li><li>Technical Skills: Proficient with software tools such as Adobe, Outlook, Teams, Word; ability to learn new EMR systems. </li><li>Communication: Excellent written and verbal communication skills. </li><li>Training Ability: Comfortable leading caregiver training sessions for groups of 5-10. </li><li>Transportation: Reliable and consistent transportation for travel within assigned territory. </li><li>Core Competencies: Strong communication, empathy, teamwork, and commitment to exceptional patient care. </li></ul>
