We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.</p> <p>Part Time 24 hours, Day Shift Mon-Friday</p> <p>Staff Nurse AMB - MNA</p> <p>Heart Transplant/ Center for Advanced Heart Disease</p> <p>The Registered Nurse Coordinator in the Center for Advanced Heart Disease (CAHD: Heart Failure, VAD, and Heart Transplant) is a professional registered nurse licensed in the state of Massachusetts with heart failure experience who is responsible for all aspects of ambulatory care of the advanced heart disease patient. This position will work closely with all members of the AHD team, inpatient staff, and others as necessary to ensure efficacious care in the ambulatory and hospital settings.</p> <p>Advanced Heart Disease patients are an extremely complex group and as such the coordinator must possess a strong clinical background. This nurse must be very organized, pay attention to detail, demonstrate the ability to coordinate and follow through on all aspects of the patient's care and effectively communicate with all members of the AHD team and the patient.</p> <p>One must demonstrate the ability to effectively manage patients by assuming full responsibility for the assessment, planning, implementation, and evaluation of patient care. The RN is directly responsible to a designated nurse director, or supervisor.</p> <p>Job Summary</p> <p>PRINCIPAL DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Supporting and extending the clinical activities of AHD physicians and nurse practitioners </li><li>Clinical evaluation, triage and needs assessment for patients referred to the ACTIVE center for IV therapy </li><li>Provision of education regarding heart failure management to patients admitted to the treatment center </li><li>Collecting and entering data into AHD databases as required by the hospital, and by local, regional, and national agencies </li><li>Provide education to patients regarding important aspects of care, including but not limited to activity, diet, medication compliance, and healthcare follow-up. </li><li>Set up consults with specialty providers, follow up on consult recommendations, and communicates results to the patient and other providers. </li><li>Assist with communication and correspondence to/from patients, employers, pharmacies, and insurance providers with regards to medication prescriptions, cardiac rehabilitation, disability, family medical leave, and payment coverage. May also communicate and correspond with other providers (e.g., utility company, telephone company) as dictated by patient needs. These duties may require telephone calls, faxes, mail, and/or e-mail. </li><li>Actively participates in the inpatient discharge process and outpatient follow up </li><li>Other duties within the CAHD as assigned by the Nurse Manager or Nurse Director </li></ul> <p>PROFESSIONAL ATTRIBUTE:</p> <ol> <li> <p>Establishes effective professional relationships with other members of the healthcare team.</p> </li><li> <p>Serves as a role model and resource for other staff, providing assistance with the care coordination process, clinical skills, and patient teaching.</p> </li><li> <p>Maintains professional relationships with appropriate professional organizations.</p> </li><li> <p>Regularly incorporates new scientific findings in a specific area of expertise into clinical practice. Maintains awareness of new developments in the specialized area and participates in research activities.</p> </li></ol> <p>PARTICIPATORY MANAGEMENT:</p> <ol> <li> <p>Utilizes time and materials effectively.</p> </li><li> <p>Participates in the orientation of new staff members, students, and others as appropriate. Provides input into the evaluation of other staff as appropriate.</p> </li><li> <p>Adheres to hospital and departmental policies. Recommends changes in policies and procedures through appropriate channels.</p> </li><li> <p>Participates in determining and implementing goals and objectives for the transplant program, and in the periodic review of the program.</p> </li></ol> <p>Qualifications</p> <p>QUALIFICATIONS:</p> <ul> <li> <ol> <li>Graduate of an approved school of nursing with current registration in Massachusetts. BSN required. </li></ol> </li><li> <ol start="2"> <li>Minimum five years of experience (within the last 8 years) with cardiology and heart failure-focused patient care required. </li></ol> </li><li> <ol start="3"> <li>Strong familiarity with diuretic therapy and inotropic therapy for heart failure patients required. </li></ol> </li><li> <ol start="4"> <li>Current (within 2 years) ambulatory heart disease/heart failure / VAD experience strongly preferred. </li></ol> </li><li> <ol start="5"> <li>Experience with telephone triage and ambulatory patient care is strongly preferred. </li></ol> </li><li> <ol start="6"> <li>Proficiency with IV placement and infusion therapy preferred. </li></ol> </li><li> <ol start="7"> <li>Knowledge of community resources and post / non-acute care providers preferred. </li></ol> </li><li> <ol start="8"> <li>Familiarity with discharge and resource planning preferred. </li></ol> </li><li> <ol start="9"> <li>Must have strong organizational and communication skills. </li></ol> </li><li> <ol start="10"> <li>Flexibility, excellent judgment, and the ability to work independently and as part of a team are essential. </li></ol> </li><li> <ol start="11"> <li>Must be able to function well in a busy environment and to manage multiple responsibilities. </li></ol> </li></ul> <p>SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:</p> <p>The RN must show evidence of the basic analytic thinking necessary to care for a group of patients. Must demonstrate observational skills and the ability to set priorities. Must be able to function under stress with good interpersonal and communication skills. Must demonstrate effective skills in applying hospital standards in area of service, team work, communication, respect for others, and time/priority management.</p> <p>WORKING CONDITIONS:</p> <p>Works in a variety of patient care environments where there may be exposure to communicable diseases and hazardous materials such as chemotherapeutic agents, radioisotopes, and radiation. Work entails walking and lifting. Required to Work Weekends Per Contract.</p> <p>SUPERVISORY RESPONSBILTIES:</p> <p>None</p> <p>HOSPITAL WIDE RESPONSIBILITIES: These are required of all staff, regardless of position.
