Primary population is program-eligible pregnant women to ensure they have a medical home for prenatal and post-partum care, practice appropriate pregnancy self-care, get connected to needed social services to remove barriers to care, and learn basic maternal and baby care with the goal of achieving a full term, healthy birth weight birth. Provides education on topics including safe sleep practices, benefits of breastfeeding, and dangers of tobacco use. Services are home-based. May also serve high needs, low-income individuals who are not pregnant.</p> <p>Preferred: Certified Community Health Worker. If no certification, will need to obtain within 12 months of hire. Required: High School diploma or GED, valid Ohio Driver's License and reliable transportation.
