<p>Clinically progressive, forward thinking outpatient clinic is recruiting for a full-time Referrals and Patient Assistance Coordinator to join our innovative hospital-based outpatient Medication Therapies Clinic. Patient care services offered include Anticoagulation, Diabetes, COPD, Asthma, Heart Failure, Transitions of Care, Weight Loss management, Cholesterol management, and Medication Therapy Management (MTM).</p> <p>What You'll Do:</p> <p>The Referrals and Patient Assistance Coordinator will support our clinical team and services helping to coordinate entry into our model of care as all patients seen at the clinic are by referral only. In our clinic, Clinical Pharmacists and Nurse Practitioners work collaboratively with referring providers utilizing consult agreements. Additionally, our clinic provides patient assistance support utilizing a combination of manufacturer assistance programs, grants, and copay assistance cards as just a few of the ways we help lower medication costs for patients. The individual who is hired for this position will serve in a key role by helping to coordinate the processes the clinic follows to help patients gain better access to their medication.</p> <p>About The Role:</p> <p>The Medication Therapies Clinic includes a team of Clinical Pharmacists, Nurse Practitioners, a Registered Nurse, and a Licensed Practical Nurse. We are looking to expand our care team with the addition of a Referrals and Patient Assistance Coordinator. Some additional duties may include limited patient rooming responsibilities after appropriate training and support from our clinical staff.</p> <p>This is an excellent opportunity to practice within an innovative outpatient clinic with a collaborative group of healthcare professionals focused on helping patients with complex chronic illnesses. We are looking for candidates with experience in outpatient care who have excellent communication and interpersonal skills and are comfortable working in a team-based environment.</p> <p>The Department of Pharmacy at Memorial Hospital is a leading innovator in the provision of clinical pharmacy services including advanced ambulatory care clinics. Equivalent combination of related education and experience will also be considered. Previous experience with Epic desired.
