<p>The HBPC RN is a member of Geriatric & Extended Care (GEC) Services, whose role is to provide case management and nursing care to a caseload of veterans in the HBPC program. Care is provided to patients in their own home with the goal of achieving quality patient care outcomes, increased continuity of care, and minimizing health care cost.</p> <p>The RN delivers fundamental, knowledge-based care to assigned clients while developing technical competencies. Such nursing care includes:</p> <ul> <li>Identifying patterns of human responses to actual or potential health problems amenable to a nursing regimen; </li><li>Executing a nursing regimen through the selection, performance, management, and evaluation of nursing actions; </li><li>Assessing health status for the purpose of providing nursing care; </li><li>Providing health counseling and health teaching;( </li><li>Administering medications, treatments, and executing regimens authorized by an individual who is authorized to practice in this state and is acting within the course of the individual's professional practice; </li><li>Teaching, administering, supervising, delegating, and evaluating nursing practice. </li><li>The Home Based Primary Care (HBPC) staff Nurse is accountable to the GEC Nurse Manager for providing high quality nursing care. This responsibility includes the utilizations of general clinical nursing skills & knowledge in direct patient care provided in the residential setting. </li><li>The RN will adhere to all current directives of VHA, and meet current Joint Commission standards, and VA-OIG expectations. </li><li>The RN will intentionally create opportunities to leverage virtual care technology to support the mission of the facility and VA at large. </li></ul> <p>A current, valid state-issued Driver's License is required for this position. NOTE: We cannot accept photographs, therefore; please do not submit a copy of your license with your application package.
