We invite you to become part of Connecticut's most comprehensive healthcare network.</p> <p>The creation of the HHC System Support Office recognizes the work of a large and growing group of employees whose responsibilities are continually evolving so that we and our departments now work on behalf of the system as a whole, rather than a single member organization.</p> <p>With the creation of our new umbrella organization we now have our own identity with a unique payroll, benefits, performance management system, service recognition programs and other common practices across the system.</p> <p>Position Summary:</p> <p>The Performance Improvement Coordinator performs systematic and objective performance improvement activities for both clinical and non-clinical areas. Responsibilities include, but are not limited to, data abstraction, analysis and reporting, and all duties as assigned including administrative support for the Quality/Regulatory/Patient Safety Team.</p> <p>Position Responsibilities:</p> <p>Key areas of responsibility</p> <ol> <li> <p>Assists in performance improvement activities to monitor the quality of care to meet regulatory standards</p> </li><li> <p>Performs specific performance improvement activities necessary for the clinical and administrative staffs to monitor and evaluate the quality and appropriateness of patient care to meet regulatory requirements (e.g., DPH, TJC, CMS)</p> </li><li> <p>Provides data abstraction from hospital applications and inputs into internal and external databases to support publicly reported measures and other performance improvement activities. Coordinates medical record audits for the state, federal, and/or managed care review</p> </li><li> <p>Interprets regulatory specifications of Core Measures and accurately oversees CMS validation requirements</p> </li><li> <p>Generates analytical reports with statistical quality control tools for specialized and generic performance improvement activities</p> </li><li> <p>Coordinates ongoing screening, collection, and analysis of information</p> </li><li> <p>Identifies problems that have an impact on patient care and the clinical performance of members of the hospital staff</p> </li><li> <p>Provides administrative support for Quality/Regulatory/Patient Safety including support for meetings: including but not limited to scheduling, coordinating agendas, taking minutes, compiling day of final presentations, distributing pre-meeting packets, copying meeting materials, preparing meeting minutes. Meetings include Regulatory, Patient Safety, PICC, QAPIC, SERT, MSEC.</p> </li><li> <p>Facilitates RAVE and internal communications for regulatory activity. Reviews and updates RAVE notification list quarterly.</p> </li><li> <p>Creates reports including EPIC daily census, OR procedure lists, restraint report and supports ongoing regulatory activities. Provides support in scanning regulatory activity files into shared drive, research policies and IFU's from online resources.</p> </li></ol> <p>Working Relationship:</p> <p>This Job Reports To (Job Title): Regional Director of Quality and Safety</p> <p>Requirements and Specifications:</p> <p>Education</p> <ul> <li>Minimum: Associate Degree (desired but not required) </li><li>Preferred: Bachelor's Degree (desired but not required) </li></ul> <p>Experience</p> <ul> <li>Minimum: One to three years of experience in the medical record or equivalent medical terminology knowledge </li><li>Preferred: Knowledge of Microsoft office products including word, excel, powerpoint ; quality improvement methods, tools and techniques </li></ul> <p>Licensure, Certification, Registration</p> <ul> <li>Preferred: CPHQ or other- optional </li></ul> <p>Knowledge, Skills and Ability Requirements:</p> <ul> <li>Strong interpersonal skills with a focus on customer service </li><li>Microsoft Office proficiency in Word, PowerPoint, Excel and Access </li><li>Able to acquire skills in data tools such as control chart, Pareto chart, trending data and analytics </li><li>Excellent written and verbal communication skills </li></ul> <p>We take great care of careers.</p> <p>With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
