<p>Title : Licensed Practical Nurse</p> <p>Reports To : Director of Nursing</p> <p>Hours : Part time/PRN Salary : Hourly wage</p> <p>Overtime : Allowed With Prior Authorization Required Evaluations : Yearly</p> <p>Education : First Aid, CPR Experience :</p> <p>Responsibilities :</p> <ol> <li>Maintains patient safety at all times. </li></ol> <ul> <li>Help provide and maintain therapeutic medium for patients. </li><li>Actively participates in treatment team and staff meetings. </li><li>Provides competent nursing care of all patients within the boundaries of licensure. </li><li>Complete admission process including nursing assessment upon admission. </li><li>Monitors for signs and symptoms of withdrawal by completing CINA scale, monitoring vital signs and mental status. </li><li>Assess physical complaints and problems making any necessary referrals to Medical Director and/or Emergency Room. </li><li>Dispense medications as ordered to patients in detox and assess for untoward effects and effectiveness of prescribed medications. </li><li>Be familiar with actions, side effects and dosage of all prescribed medication. </li><li>Supervise self-administration of medications. </li><li>Maintain accurate records of Scheduled Drugs. </li><li>Demonstrate good nursing judgement in dispensing ordered prescribed and over-the-counter medication. </li><li>Perform veni-punctures using acceptable technique with proper preparation of sample as indicated by test ordered. </li><li>Obtain urine samples for drug screens as ordered. </li><li>Ability to perform in-house drug screens using the ADX analyzer and routine maintenance procedures with proper instruction. </li><li>Collect and prepare samples as ordered for cultures and sensitivity. </li><li>Assist Medical Doctor with physical examinations. </li><li>Develops, implements and maintains current nursing care plans for assigned patients. </li><li>Inventory supplies emergency box, first aid kit and medications and notify nursing coordinator/manager of needs. </li><li>Accurate implementation of physician orders. </li><li>Patient teachings as indicated by special needs, demonstrated lack of knowledge and assessments. </li><li>Thorough and accurate documentation of patient care, procedures, complications and progress in medical record using the problem oriented record system. </li><li>Orient patients to program and phases of treatment. </li><li>Prepare and deliver lectures and in-services as requested within the scope of knowledge and training. </li><li>All duties and responsibilities as assigned within the boundaries of licensure, knowledge, training, working conditions and physical demands. </li></ul>
