<p>Job Functions:</p> <ul> <li>Guides CHF patients through inpatient, discharge, and outpatient phases of care to ensure continuity and timely access to needed services. </li><li>Serves as the central point of contact and liaison between patients, families, and members of the multidisciplinary HVI team. </li><li>Provides education on CHF disease processes, medications, lifestyle modifications, symptom management, and follow-up care. </li><li>Engages patients in shared decision making in alignment with American College of Cardiology (ACC) guidelines and supports adherence to Guideline Directed Medical Therapy (GDMT). </li><li>Participates in clinical discussions, HVI conferences, and coordination activities that support integrated patient care. </li><li>Builds and maintains effective working relationships with physicians, APPs, nurses, and other care providers. </li><li>Assists with data collection for registries, performance monitoring, and quality improvement initiatives related to CHF outcomes and care processes. </li><li>Contributes to efforts aimed at improving patient experience and clinical effectiveness within the HVI Service Line. </li><li>Maintains UpToDate knowledge of CHF therapies, treatment guidelines, and navigation best practices. </li><li>Demonstrates competency in care coordination, patient education, communication, and professionalism. </li><li>Completes all required annual education and training. </li><li>Represents McLeod Health and the HVI Service Line professionally in both internal and community settings. </li><li>Participates in community outreach events as requested. </li><li>Performs other duties as required. </li></ul> <p>Qualifications /Training:</p> <ul> <li>A minimum of 3 years of recent cardiovascular experience. </li><li>Strong knowledge of heart failure pathophysiology, GDMT medications, and other evidence-based guidelines. </li><li>Excellent communication, patient education, and care-coordination skills. </li><li>Ability to work independently and collaboratively across multidisciplinary teams. </li></ul> <p>Licenses/Certifications/Registrations/Education:</p> <ul> <li>Current South Carolina (or compact state) licensure as RN (SCLLR) in good standing required. </li><li>Basic Life Support (BLS) certification required. </li><li>Advanced Certification as approved by VP of Cardiovascular Services within 3 years of hire </li></ul> <p>Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses.
