<p>University of Iowa Health Care Office of Population Health & Accountable Care Organization (ACO) Operations is seeking a Nurse Clinician to join our team. Our team works to change the way care is delivered with the goal of improving quality, lowering cost, and reducing health disparities for all Iowans. We accomplish this in a multidisciplinary approach where our nursing staff are valued members of the team. We are seeking a welcoming, highly skilled, and collaborative person to support and enhance patient care through population health initiatives that drive quality and the Quadruple Aim.</p> <p>The Population Health Nurse Clinician functions as a clinical nurse expert and care manager for specific patient populations, collaborating with care teams, patients, and families. They will manage a specified population and work closely with other care coordinators, as well as other interdisciplinary team members across the continuum of care to provide care management. They will use knowledge of public health concepts to build and support new programs and initiatives that drive clinical innovation and care redesign to move care upstream and manage health at the population level. For example, collaborating with community partners to provide an immunization clinic for a target population.</p> <p>This position is eligible for hybrid-remote work following the completion of probationary period and successful orientation. Remote work must be within Iowa and will require a work arrangement form to be completed upon the start of your employment. Per policy, work arrangements will be reviewed annually, and must comply with the remote work program and related policies and employee travel policy when working at a remote location.</p> <p>Position Responsibilities</p> <ul> <li>Collaborates with nursing, medical staff and other health professionals in planning, implementing, and evaluating patient care for a specific patient population with regard to the condition, age, and risk factors of the patient. </li><li>Independently coordinates and facilitates patient care between multiple disciplines within both UI Health Care and local communities. Facilitate continuity of care. Educate and support patients and their families regarding health promotion, disease prevention, health maintenance, and chronic disease management, ensuring educational materials utilize plain language principles and are culturally appropriate. </li><li>Revises the patient plan of care via the telephone or telemedicine within protocols developed collaboratively. </li><li>Assess social drivers of health (SDOH) needs for patients using a standardized screening tool, provide referrals, and facilitate access to community services, such as food assistance, transportation, social support, etc. </li><li>Develop and maintain a comprehensive understanding of UI Health Care and community resources and services available to address a patient's health and SDOH needs. </li><li>Participates in development and implementation of managed care programs for a specific patient population. </li><li>Collaborate with care teams to develop, implement, and support targeted best practice initiatives to address quality outcomes, health disparities, ED utilization, hospitalization, and length of stay. </li><li>Serve as a clinical resource for specific patient populations with regard to the condition, age, and risk factors of the patients. </li><li>Collaborate with clinic-based nursing team(s). </li><li>Participates in/provides leadership for divisional, departmental and/or intra institutional and community committees and projects. </li><li>Incorporates the role of patient care provider, advocate (patient, family, and health care professional), consultant, educator, researcher and quality manager. </li><li>Uses relevant nursing research to improve clinical practice and investigate nursing care problems related to a specific patient population. </li><li>Supports implementation and evaluation of staff education and orientation. </li><li>Act as a preceptor to provide educational experiences for health care students. </li><li>Perform other special projects and tasks as assigned. </li><li>Assumes responsibility for maintaining clinical expertise consistent with patient population requirements and current nursing and medical practice. </li><li>Adheres to professional and legal standards of nursing practice. </li><li>Maintains confidentiality of patient and staff information. </li><li>Autonomously communicate and interact with care teams, colleagues, and patients in a professional and cooperative manner. </li><li>Promotes a positive work environment. </li><li>Demonstrates the core values of service leadership: respect, communication, accountability, integrity, and compassion. </li><li>Remains calm and constructive in conflict situations, listens and attempts to understand other points of view. </li><li>Analyzes own performance in accordance with position expectations, develops goals and implements a plan to meet goals. </li><li>Is self-directed in using time. </li></ul> <p>Percent of Time: 100%</p> <p>Schedule: 40 hours, Monday - Friday</p> <p>Location: UI Health Care HSSB</p> <p>SEIU Pay Plan: https://hr.uiowa.edu/pay/pay-plans/seiu-pay-plan.</p> <p>Benefits Highlights:</p> <ul> <li>Regular salaried position located in Iowa City, Iowa </li><li>Fringe benefit package including paid vacation; sick leave; health, dental, life and disability insurance options; and generous employer contributions into retirement plans </li><li>For more information about Why Iowa?, click HERE! </li></ul> <p>Education Requirements</p> <ul> <li>Baccalaureate degree in Nursing required. </li><li>Current license to practice nursing. </li></ul> <p>Experience Requirements</p> <ul> <li>Reasonable (within the last 1-3 years) nursing experience is required. </li><li>Demonstrated expertise in specific clinical area of practice required. </li></ul> <p>Desired Qualifications</p> <ul> <li>Experience in care management or care coordination. </li><li>Experience in community health or public health settings. </li><li>Experience in behavioral health. </li><li>Demonstrate job-related experience with fostering a welcoming and respectful work/academic environment and/or otherwise demonstrate their ability to foster a welcoming and respectful work/academic environment for people from all types of backgrounds and viewpoints </li><li>Knowledge of chronic disease management. </li><li>Experience applying motivational interviewing principles to effectively engage and motivate patients. </li><li>Experience in program development. </li><li>Certification in care coordination, care management, or related area. </li></ul> <p>Please attach a cover letter and resume as part of the application process.
