<p>Job Summary</p> <p>Under the general direction of the hospital pharmacy, Medication Collection Pharmacy Technicians:</p> <ul> <li>Independently perform tasks related to collecting, reconciling, and entering the home medication history into the patient's electronic medical record. </li><li>Interview the patient, patient family members, call local pharmacies and use other available resources for the purpose of compiling a medication history. </li><li>Compile and enter medication history information into the patient's electronic health record for reconciliation by the admitting provider and use by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. </li><li>Interact with and provide medication history and other related patient information to nurses, physicians, pharmacists, and other medical professionals as appropriate.
