Establish, implement, and adapt an individualized treatment plan of care (including appropriate goal setting, treatment activities and frequency). </li><li>Document accurately, completely, and timely. </li></ul> <p>What you will need:</p> <ul> <li>Demonstration of at least entry level professional competency of Occupational Therapy practice, theory, principles and applications </li><li>Bachelors Degree required </li><li>NYS OT license are required </li><li>Must function well within a team concept of rehabilitation professionals. </li><li>Excellent communication skills, flexibility and exceptional clinical reasoning skills are essential. </li></ul> <p>Log onto http://www.sphp.com/working-at-sunnyview-svh to see why Sunnyview is an amazing place to work!</p> <p>Pay Range: $35.85-$45.50 (Per DIEM RATE IS FLAT RATE OF $50.00/hour)</p> <p>Pay outside of Per Diem rate is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
