<p>ProMedica Physicians Group (PPG) is seeking a dedicated and compassionate Advanced Practice Provider to join our healthcare team in Perrysburg, Ohio, part of the Greater Toledo area.</p> <p>HIGHLIGHTS:</p> <ul> <li>Schedule: Monday - Friday 8:00am - 4:30pm </li><li>Weekend/ Call Requirements: Rotating coverage once a month </li><li>Current team consists of 7 providers </li><li>Support Staff: RN Care Navigator, MA's, PCS's, and Practice Director </li><li>Deliver comprehensive medical care to patients 18+ years of age, including conducting physical examinations, diagnosing and treating acute and chronic conditions, and prescribing medication </li><li>Develop individualized treatment plans based on patient history and medical assessments while collaborating with a multidisciplinary team to ensure high-quality, coordinated care. Build a practice and work in silo with other providers patients to optimize access for patients </li><li>Play a key role in educating patients on preventive care, healthy lifestyle choices, and disease management, contributing to the overall well-being of the community.
