<p>Job Description</p> <p>Overview</p> <p>Social Worker - MSW LSW - PRN</p> <p>Location: Methodist Hospital</p> <p>Schedule: Primarily Monday through Friday, 8:00 AM - 4:30 PM, with rotating weekends (every 4th weekend) and holiday rotations</p> <p>Position Overview:</p> <p>The PRN Social Worker meets face-to-face with patients and families to assess needs, provide counseling, and coordinate care. The role involves collaborating with all members of the healthcare team to ensure patients receive timely, safe services, and families are connected with appropriate resources and education. The Social Worker also serves as a liaison with protective services and supports complex discharge planning, crisis intervention, and mental health assessments.</p> <p>Key Responsibilities:</p> <ul> <li> <p>Meet face-to-face with patients and families to conduct psychosocial assessments and provide counseling.</p> </li><li> <p>Collaborate with healthcare teams to facilitate the episode of care, ensuring safe and timely services.</p> </li><li> <p>Interpret and communicate complex patient and family needs, role modeling caring practices to other caregivers.</p> </li><li> <p>Assess psychosocial risk factors through evaluation of prior functioning, support systems, reactions to illness, and coping skills.</p> </li><li> <p>Counsel patients and families regarding emotional, social, and financial impacts of illness/disability.</p> </li><li> <p>Access and mobilize family and community resources to meet identified needs.</p> </li><li> <p>Provide mental health assessments of patients upon physician referral.</p> </li><li> <p>Deliver therapeutic interventions to support complex discharge planning.</p> </li><li> <p>Serve as the point person for cases involving child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence.</p> </li><li> <p>Act as liaison between Indiana University Health and local child and adult protective teams (APS/DCS).</p> </li><li> <p>Provide intervention in guardianship (temporary/permanent), foster care, adoption, and mental health Level II placements.</p> </li><li> <p>Offer specialized mental health services, including crisis intervention and addiction assessments, with recommendations for psychiatric or community resources.</p> </li><li> <p>Assist during dangerous or potentially dangerous situations, taking a leadership stance to provide clinical intervention.</p> </li><li> <p>Support individuals experiencing acute crises, utilizing de-escalation techniques to prevent violence.</p> </li><li> <p>Use effective interviewing principles with victims, perpetrators, families, and other significant persons.</p> </li><li> <p>Provide intervention for end-of-life, guardianship, foster care, adoption, and related cases.</p> </li></ul> <p>Qualifications & Requirements:</p> <ul> <li> <p>Education: Master's Degree in Social Work required.</p> </li><li> <p>Licensure: Current Indiana LSW licensure required; permanent licensure preferred.</p> </li><li> <p>Certifications:</p> </li><li> <p>Crisis Prevention Intervention (CPI) or an alternative de-escalation training required within 6 months of hire.</p> </li><li> <p>Knowledge & Skills:</p> </li><li> <p>Evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention techniques.</p> </li><li> <p>Knowledge of disease processes and human behavior related to health social needs.</p> </li><li> <p>Complex discharge planning skills to ensure safe patient transitions.</p> </li><li> <p>Legal mandates concerning child and adult protective services at the state and county levels.</p> </li><li> <p>Principles of growth, development, and end-of-life care.</p> </li><li> <p>Ability to identify decision-makers and facilitate goals-of-care conversations.</p> </li><li> <p>Experience adapting services to geographically, culturally, and financially diverse populations.</p> </li><li> <p>Ability to develop and maintain strong relationships with hospital leadership, medical teams, law enforcement, APS/DCS, schools, probation, and community organizations.</p> </li><li> <p>Computer literacy and proficiency in documentation within an electronic medical record (EMR).</p> </li><li> <p>Ability to respond appropriately to violent or potentially violent situations, including de-escalation.</p> </li></ul> <p>Core Competencies:</p> <ul> <li> <p>Excellent communication and active listening skills.</p> </li><li> <p>Strong crisis intervention and de-escalation techniques.</p> </li><li> <p>Ability to collaborate within a multidisciplinary team.</p> </li><li> <p>Maintaining confidentiality and HIPAA compliance.</p> </li><li> <p>Cultural sensitivity and professionalism.</p> </li></ul> <p>Additional Information:</p> <ul> <li>May be scheduled for on-call and holiday rotations as needed. </li></ul> <p>Join our team and contribute to providing high-quality psychosocial support to patients and families in a dynamic healthcare environment.</p> <p>To learn more about our benefits, visit: IU Health Benefits</p> <p>Interested?</p> <p>Apply today to become a PRN Social Worker - MSW, LSW with Indiana University Health!</p>
