<p>The MDS/Enrollment Nurse is responsible for ensuring potential and or current PACE participants are meeting the clinical requirements according to the Minimum Data Set clinical (MDS) assessment. The individual in this role conducts home visits and assists in completing the intake Prospect sheet for visits. The MDS Nurse is responsible for managing all aspects of the clinical assessment. the MDS/Enrollment Nurse collaborates with other departments to provide clinical information.</p> <p>Pay Range: The expected base pay for the position is $36-$65.02 per hour and may be increased based on other factors, such as language, certifications, etc.</p> <p>Duties & Responsibilities:</p> <ul> <li>Conducts home visits to provide clinical assessment for prospective participants. </li><li>Performs all necessary actions to complete initial and annual state required clinical assessments. </li><li>Provides clinical information to the IDT and contributes to the completion of the initial MDS and provisional care plan, acts as a liaison between PACE and the pre-enrollees current home services. </li><li>Submits completed MDS to EOEA/OLTC. </li><li>Completes reassessment and submission of expired clinical assessments. </li><li>Assures presence of completed and signed Health Care Proxy, Medical Record Form and other designated forms. </li><li>Provides detailed clinical information of potential applicants to IDTs to assist them in preparing for transition of new enrollees. </li></ul> <p>Other Duties, Responsibilities & Organizational Competencies:</p> <ul> <li> <ol> <li>Adheres to UCHC, Inc. standards of Employee Conduct and all related policies and procedures by: - Demonstrating excellent customer service; - Showing respect to other employees; - Adhering to standards of cultural competency and non-discrimination; - Adhering to standards of privacy; - Adhering to attendance and other policies and utilizing appropriate notification and other procedures. </li></ol> </li></ul> <ol start="2"> <li> <p>Upholds procedures and systems to safeguard the confidentiality of all patient and employee information.</p> </li><li> <p>Upholds safety policies, practices, and procedures including safety, fire safety, electrical safety, proper body mechanics and material handling, office/ergonomic safety and other employee safety measures.</p> </li><li> <p>Attends PACE and other required trainings annually.</p> </li></ol>
