We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.</p> <p>We are seeking a dedicated and experienced Clinical Pharmacist to join our Integrated Care Management Program (iCMP), supporting a high-risk, medically complex patient population as a member of a Care Team. This role offers an opportunity to be at the forefront of value-based, team-driven care, applying your clinical expertise to improve health outcomes and reduce hospitalizations through medication optimization.</p> <p>You'll collaborate with physicians, nurses, social workers and care managers to develop and implement patient-centered care plans focused on chronic disease management, medication safety, and affordability.</p> <p>While this role includes a hybrid work structure, the emphasis is on meaningful patient care and interdisciplinary collaboration-with occasional in-person meetings held at our Assembly Row location in Somerville, MA.</p> <p>Why Join Us?</p> <ul> <li>Make a Difference: Improve outcomes for vulnerable patients through targeted clinical interventions. </li><li>Team-Based Environment: Work alongside a multidisciplinary group committed to coordinated, high-quality care. </li><li>Professional Growth: Engage in quality improvement initiatives and contribute to innovative care models. </li><li>Balanced Flexibility: Enjoy a predominately remote schedule with periodic in-person meetings and team activities in Somerville. </li></ul> <p>Job Summary</p> <p>Key Responsibilities:</p> <ul> <li>Conduct Comprehensive Medication Management (CMM) and medication reconciliation, especially during care transitions. </li><li>Provide chronic disease management through Collaborative Drug Therapy Management (CDTM) agreements. </li><li>Identify and resolve issues related to medication safety, adherence, and cost-effectiveness. </li><li>Engage directly with patients-primarily via phone or video-to deliver counseling and education. </li><li>Partner with care teams to develop, document, and monitor personalized care plans using national guidelines. </li><li>Provide expert consultation on pharmacotherapy to medical groups and care teams. </li><li>Participate in program-level quality improvement initiatives. </li><li>Maintain up-to-date and accurate documentation in the Epic EMR system. </li></ul> <p>Qualifications</p> <p>Education & Credentials</p> <ul> <li>Doctor of Pharmacy (PharmD) from an accredited institution </li><li>Active pharmacist license in the Commonwealth of Massachusetts </li><li>Board certification required (e.g., BCACP, BCPS, BC-ADM) strongly preferred </li></ul> <p>Experience</p> <ul> <li>At least 2 years of clinical pharmacy experience, ideally in ambulatory care or managed care settings strongly preferred </li><li>Strong understanding of chronic condition management (e.g., diabetes, hypertension, COPD, CKD) </li></ul> <p>Additional Job Details (if applicable)</p> <p>Remote Type</p> <p>Hybrid</p> <p>Work Location</p> <p>399 Revolution Drive</p> <p>Scheduled Weekly Hours</p> <p>40</p> <p>Employee Type</p> <p>Regular</p> <p>Work Shift</p> <p>Day (United States of America)</p> <p>EEO Statement:</p> <p>Mass General Brigham Community Physicians, Inc. is an Equal Opportunity Employer.
