If you thrive in a fast-paced setting, value continuous learning, and enjoy working with a diverse population, we'd love to hear from you.</p> <p>Full-time position</p> <p>Supportive team environment</p> <p>Opportunities for professional growth</p> <p>Make a lasting impact every day</p> <p>Apply now to join a healthcare team that truly cares - for patients and for each other.</p> <p>Compensation and Benefits Offered:</p> <ul> <li>Starting pay $28.00 an hour - goes up based on experience </li><li>Paid Time Off (PTO) - Accrued per pay </li><li>Insurance (Medical, Dental, Vision, and Life) </li><li>Paid Holidays - 7 paid holidays </li><li>403b Retirement with up to 8% match (starts at 3% and increases with time of service at HPWO) </li><li>Annual Reviews and Increases </li><li>Mileage Reimbursement - Work related travel </li><li>Employee Assistance Program </li><li>Referral Bonus - Earn more by expanding our team </li><li>Training Opportunities </li><li>Eligible to apply for the Emerging Leaders Program after 1 year of service </li></ul> <p>Qualifications:</p> <p>Graduate of a school of nursing accredited by National League of Nursing.</p> <p>Current license to practice as a Licensed Practical Nurse in the State of Ohio.</p> <p>Skills/Abilities:</p> <p>CPR - BLS (basic life support) required.</p> <p>One to three years full-time experience in nursing practice is preferred.</p> <p>Ability to work as a team member.</p> <p>Computer experience desirable.</p> <p>Ability to work with a culturally diverse group of people.</p> <p>Must be willing to work flexible hours as required by position.</p> <p>Essential Functions and Basic Duties:</p> <ul> <li>Collects patient data through physical examination techniques, interview, and observation. </li><li>Documents all nursing care provided according to policy. </li><li>Formulates plan of care for clients under direction from providers. </li><li>Obtains referrals from other physicians and specialists. </li><li>Administers medications as prescribed by the provider. </li><li>Follows approved protocols/standing orders for client care. </li><li>Follow universal precautions. </li><li>Performs treatments, procedures as prescribed by the provider. </li><li>Performs technical skills. </li><li>Responds to client/family/caregiver needs. </li><li>Responds to crisis situations according to policy. </li><li>Utilizes client education for emotional and physical health maintenance. </li><li>Utilizes supplies in a cost-effective manner. </li><li>Performs clinic and outreach related tasks. </li><li>Utilizes on-going data collection skills. </li><li>Participates in Quality Improvement activities. </li><li>Demonstrates effective communication skills. </li><li>Establishes and maintains professional working relationships with other members of the health care team. </li><li>Participates in planning and problem solving. </li><li>Participates in continuing education and professional growth. </li><li>Attends staff meetings. </li><li>Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures. </li><li>Assists with seasonal programs as needed. </li><li>Assists other team members. </li><li>Performs nursing visits. </li><li>Completes patient health screenings including hearing, vision, and all CLIA waived testing. </li><li>Maintains stocks of medicines and medical supplies as necessary. </li><li>Educates and advises patients on specified medical issues within established parameters. </li><li>Performs patient education and discharge instructions. </li><li>Completes referrals and schedules tests for patients. </li><li>Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices. </li></ul> <p>Work Environment:</p> <p>Work is primarily sedentary, demanding sitting, walking, lifting, and bending. Those physical movements and the degree of mobility, manual dexterity, and hand-eye coordination commonly associated with duties in an office setting will be performed repetitively. This also includes bending, twisting, reaching, lifting, pulling, pushing, and walking. The ability to distinguish letters and symbols and utilize telephones, computer terminals, fax machines, and copiers is required. The work environment characteristics described here represent those an employee encounters while performing the essential functions of this job.
