Together, we can make a significant difference in the lives of children and contribute to a brighter and healthier future for all.</p> <p>Summary:</p> <p>Children's Health Medical Group (CHMG) Primary Care clinics may utilize and hire Licensed Vocational Nurses (LVN) as part of their staffing model. This position works under the general supervision of an RN, APRN, Physician, Physician Assistant, Podiatrist, or Dentist to provide safe, compassionate, and focused nursing care to assigned patients with predictable healthcare needs. Works within their scope of practice as defined by the Board of Nurse Examiners for the State of Texas. This position may not be used in an inpatient setting.</p> <p>Responsibilities:</p> <ul> <li>All medical acts performed by a Licensed Vocational Nurse are pursuant to delegation by and under the supervision of an RN, APRN, Physician, Physician Assistant, Podiatrist, or Dentist. The delegating Physician remains responsible for the medical acts of the LVN performing those acts. Subject to this delegation requirement, the following job-specific responsibilities apply: </li><li>Demonstrates knowledge and competency in current vocational nursing theory; provides direct patient care as listed on LVN competency checklist; recognizes and communicates changes in patient condition or needs to Provider or RN; and contributes to care plan and discharge planning. </li><li>Must use clinical reasoning and established evidence-based policies, procedures, or guidelines as the basis for clinical judgment in nursing practice. </li><li>Collects information and data that can assist Provider or RN in triaging patient phone calls. </li><li>Performs health teaching with patients and families, based on medical and nursing care plans; provides appropriate emotional support to patients and families; collaborates with other health team members to ensure continuity of patient care; and demonstrates sensitivity to the needs and satisfaction of patients, families, and other customers. </li><li>Assists in maintaining the adequacy, orderliness, and safety of the unit's/department's environment, equipment, and supplies. </li><li>Participates in the unit's performance improvement activities. Seeks and utilizes efficient and effective methods for performing job duties; utilizes resources appropriately; demonstrates cooperation and teamwork in carrying out assignments; and performs other duties and special projects as required. </li><li>Will deliver care with a team orientation, an emphasis on good customer relations, sound clinical judgment, and appropriate decision-making abilities that take into consideration evidence based practice. </li><li>Continuously inquires about the patient's condition through the ongoing process of gathering data, evaluating the situation, and implementing treatment changes, if necessary, through collaboration with the health care team, including the patient and family. </li><li>Escorts or transports patients to exam rooms, treatment rooms, or other areas as directed. </li><li>Ensure that Patient Health Information (PHI) is protected and secured at all times. </li><li>Adhere to all infection prevention initiatives (i.e., hand hygiene, proper waste disposal, etc.) </li><li>Deliver competent and skilled care to patients and families according to their identified needs. </li><li>Perform point of care testing as required for their position and completed competency. </li><li>Prepares examination rooms for patient care, facilitating use of diagnostic equipment. </li><li>Perform EKG's (as required for their Department,) by Provider order and completed competency. </li><li>Prepares and administers medications as per policy and Provider order. </li><li>Provide patients and families with pre-printed instructions and educational material. </li><li>Provide lab, imaging, and pathology results to patient and families at the Provider's request and direction. </li></ul> <p>How You'll Be Successful:</p> <p>WORK EXPERIENCE</p> <ul> <li>At least 1 year Job related experience Required </li></ul> <p>EDUCATION</p> <ul> <li>High school diploma or equivalent Required </li><li>Graduation from an accredited vocational (practice) nurse program Required </li></ul> <p>LICENSES AND CERTIFICATIONS</p> <ul> <li>Current license to practice vocational nursing in the State of Texas Required </li><li>Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements Required </li><li>PACES - New employees must obtain PACES certification within 6 months of hire Required </li></ul> <p>A Place Where You Belong</p> <p>We put our people first.
