Your recruiter will provide the most up-to-date details during the hiring process.</p> <p>How you'll make an impact in this role</p> <ul> <li>Reduce Readmissions & Ensure Safety: Oversee complex discharge plans, ensuring patients have crucial resources at home, such as housing, follow-up care, and medications. </li><li>They tackle non-medical issues and address the root causes: referred to as Social Determinants of Health (SDOH)-like food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment. </li><li>Provide Emotional Support: Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services. </li><li>Provide advocacy & resource expertise: Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge. </li></ul> <p>What minimum requirements you'll need</p> <p>Licensure / Certification / Registration:</p> <ul> <li> <p>One or more of the following required:</p> </li><li> <p>Clinical Counselor credentialed from the Oklahoma State Board of Behavioral Health obtained prior to hire date or job transfer date.</p> </li><li> <p>Counselor credentialed from the Oklahoma State Board of Behavioral Health obtained prior to hire date or job transfer date.</p> </li><li> <p>Social Worker credentialed from the Oklahoma State Board of Licensed Social Workers obtained prior to hire date or job transfer date.</p> </li><li> <p>Social Worker specializing in Clinical Practice credentialed from the Oklahoma State Board of Licensed Social Workers obtained prior to hire date or job transfer date.</p> </li><li> <p>Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.</p> </li><li> <p>Case Manager credentialed from the American Case Management Association (ACMA) preferred.</p> </li></ul> <p>Education:</p> <ul> <li>Master's degree in Social Work required. </li></ul> <p>Work Experience:</p> <ul> <li>3 years of experience preferred. </li></ul> <p>What additional preferences we're seeking</p> <ul> <li>LMSW or LCSW strongly preferred </li><li>Case Management experience a plus </li></ul> <p>Equal employment opportunity employer</p> <p>Ascension provides Equal Employment Opportunities (EEO) to all associates and applicants for employment without regard to race, color, religion, sex/gender, sexual orientation, gender identity or expression, pregnancy, childbirth, and related medical conditions, lactation, breastfeeding, national origin, citizenship, age, disability, genetic information, veteran status, marital status, all as defined by applicable law, and any other legally protected status or characteristic in accordance with applicable federal, state and local laws.
