<p>Social Worker (LMSW or LLMSW)</p> <p>Location Berkley & Novi. Split Position Supporting two locations.</p> <p>Novi - 39525 West 14 Mile Rd. 48377</p> <p>Berkley - 2361 Coolidge Hwy. 48072</p> <p>Full-Time/Days - 36 Hours per week</p> <p>Eligible for Full Time Benefits</p> <p>No weekends or on-call required</p> <p>Eligible for $10,000 Sign-On Bonus</p> <p>WHO WE ARE:</p> <p>Greenfield Health Systems' dialysis facilities (Greenfield Health Systems) offer a compassionate, multi-specialty team, modern technology, clinical expertise, and the latest medical knowledge to provide renal patients the highest quality care available. We have been part of the community since 1983 and are close to celebrating almost 40 years of excellence dedicated to dialysis!</p> <p>WHAT WE ARE LOOKING FOR:</p> <p>Masters-prepared Social Workers to work with our renal patients and families to assist with the adjustment to what is likely a life-changing event. Provide emotional and social adjustment counseling, assistance with removing barriers to treatment, education on kidney disease and treatment goals, and bridging barriers to referral services and community resources.</p> <ul> <li>Conducts initial psychosocial interviews with patients and families in order to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual/family history upon current and future psychosocial functioning. </li><li>Provides ego-supportive counseling to patients and their families in order to maximize ongoing psychosocial functioning. </li><li>Provides crisis intervention to patients and families as needed. </li><li>Functions as a patient advocate and as a liaison between the patient and multiple care delivery systems. </li><li>Participates in comprehensive inter-disciplinary team patient care meetings with appropriate documentation on IDT assessment and plan of care. </li><li>Records psychosocial assessments, update assessments and progress notes in accordance with established recording guidelines. </li></ul> <p>Education/Experience</p> <ul> <li>Masters of Social Work Degree required. </li><li>Experience in a healthcare setting preferred. </li><li>Demonstrates understanding of the impact of Chronic Kidney Disease (ESKD) on activities of daily living, quality of life and mental health needs. </li><li>Familiar with both clinical and case management techniques. </li></ul> <p>Licensure/Certification</p> <ul> <li>Must be licensed in the State of MI (LLMSW or LMSW). </li></ul> <p>Additional Information</p> <ul> <li>Organization: Community Care Services </li><li>Department: Novi Home Dialysis </li><li>Shift: Day Job </li><li>Union Code: Not Applicable </li></ul>
