At New Perspective you're not just an employee, you are a valued member of our team.</p> <p>Position Summary:</p> <p>The Clinical Assessment Nurse is a Registered Nurse (RN) who will conduct accurate and thorough comprehensive resident assessments and develop resident service plans that are individualized based on resident assessed needs. The Clinical Assessment Nurse will schedule assessments in collaboration with designated community leadership. The Clinical Assessment Nurse will provide ongoing oversite and education with community nurses on comprehensive assessment and service plan completion. The Clinical Assessment Nurse will assist with electronic health record configuration and assist with development and changes to comprehensive assessments and service plans as warranted.</p> <p>Position is based in the New Perspective Resource Center (Minnetonka, MN) and supports all Minnesota communities. Estimated time in communities is up to four days per week. Depending on point of origin, overnight travel may be required.</p> <p>Responsibilities:</p> <ul> <li>In communities across MN five days a week, assisting with the completion of comprehensive assessments and service plans </li><li>Assist with assessment-focused audits to determine accuracy and identify training needs </li><li>Provide education with new and seasoned nurses on assessment completion, as warranted </li><li>Collaborates with various roles (Clinical Reimbursement Managers, Clinician Educator, Sr. Director Quality Assurance, Clinical Leadership, etc.) </li><li>Timely report opportunities identified for implementing process improvement measures to support accurate completion of comprehensive assessments and service plans. </li><li>In collaboration with the Sr. Director, Quality Assurance, develop quality improvement measures in response to identified opportunities in assessment and service plan completion to support accurate and timely completion. </li><li>Position based in the RC and supports all Minnesota communities. Depending on point of origin, overnight travel may be required. </li><li>Other duties as assigned. </li></ul> <p>Qualifications:</p> <ul> <li>Holds and maintains a Minnesota registered nurse license in good standing. </li><li>Proficient and detailed clinical assessment and documentation skills with 1-2 years of experience working as a registered nurse with recent/current experience in completing comprehensive assessments and service/care plan development. </li><li>Experience working in electronic health records required. </li><li>Strong critical thinking and analytical reasoning skills and clinical judgment </li><li>Strong interpersonal and organizational skills </li><li>Ability to work independently as well as collaboratively. </li><li>Proficient in written and verbal communication, with the ability to clearly communicate with residents, prospective residents, families, and healthcare professionals. </li></ul> <p>Work Environment:</p> <ul> <li>Will work in a healthcare environment with potential exposure to infectious diseases transmitted via blood and bodily fluids and respiratory droplets. </li><li>The noise level in the work environment is usually moderate. </li><li>While performing the duties of the job, the employee will perform activities that will require lifting of objects up to 35 pounds and occasionally up to 50 pounds. </li></ul> <p>Salary Range: $80,000 - $90,000 based on experience</p> <p>Team Member Benefits & Perks*</p> <ul> <li>Medical, Dental, & Vision Insurance </li><li>401(k) with Company Match! </li><li>Paid Time Off and Holidays </li><li>Company-Paid Basic Life Insurance </li><li>Voluntary Short-Term Disability </li><li>Company-Paid Long-Term Disability </li><li>Health Reimbursement Account/Health Savings Account </li><li>Flexible Spending Accounts </li><li>Education assistance - up to $5,000 per calendar year! </li><li>Leadership Development & Career Advancement </li><li>Real-time Access to Earned Wages </li><li>Referral Bonuses </li><li>Employee Assistance Program </li><li>Benefits vary by full-time, part-time, and PRN status. </li></ul> <p>New Perspective is an Equal Opportunity Employer.</p>
