<p>Job Details</p> <p>Physician Office | Kettering | Full-Time | First Shift</p> <p>Responsibilities & Requirements</p> <p>Ability to provide extended patient and family therapy related to cancer treatment.Assist patients in navigating the healthcare system and accessing necessary resources.Understanding of and ability to manage patients with high distress screen/score and complex mental, physical, emotional, and financial health issues.Participates in or assists with grant writing and maintenance of grant funding related to program support services.Provides support to food pantry including fundraising and food collection/distribution.Maintains familiarity with a wide range of transportation resources including managing grant funded resources and services; able to triage transportation options to maximize organizational resources.Ability to provide in-depth assistance with financial resources, insurance, disability, and work-related issues over the course of cancer treatment.Participates in the provision of education to the health setting and community regarding oncology specific social issues.Participates in leading support groups for a variety of oncologic diseases and age groups.Provide education on and assist with Advanced Directives</p> <p>Preferred Qualifications</p> <p>Minimum Education</p> <p>Bachelor's degree in social work - required</p> <p>Minimum Work ExperienceClinical experience in health care settings and/or community agencies - preferredOutpatient health care experience - preferredExperience with oncology population - preferred.</p> <p>Required LicensesLicensed Clinical Social WorkerLicensed Social Worker in the State of Ohio - required.OSW-C (certified Oncology Social Worker) preferred</p> <p>Required SkillsExcellent communication skillsExhibits a positive attitude.Empathetic and compassionate approach to patient careFlexible, open-minded, and adaptable to change.Able to work well under pressure and maintain a professional demeanor under adverse circumstances.Knowledgeable about federal and state programs that provide medical care and support to individuals.Knowledgeable about community resources Ability to work with diverse populations.Strong crisis intervention skills Awareness of developmental needs across the life span.Ability to work collaboratively with multi-disciplinary teams to achieve optimal outcomes.Promotes individual growth and development; meets mandatory/minimum educational requirements.Ability to problem-solve and work independently.Ability to network with colleagues and community agencies.Proficient with Microsoft Office Programs and Epic.</p> <p>New Hire/Annual CompetenciesCompletion of annual mandatory and new hire competenciesCompletion of assigned social work specific education, as required annually</p>
