Veteran-specific information is included if being prepared patient specific. </li><li>Inspects packaged items for accuracy and suitability before submitting for final check, prior to distribution to hospital areas. </li><li>Maintains replenishment of pharmacy medication repackaging automation as required and follows established procedures. </li><li>Fills unit-dose carts for patient care independently without second review. </li><li>Compounds and/or reconstitutes oral or external medications (non-sterile), following established compounding records. This includes calculating and converting to proper weights and measures the amount of drug or solution to be used. Ensures final check by pharmacist prior to product being released from the pharmacy. </li><li>Assures drugs and supplies are appropriately stored and secured according to USP and VA guidelines, including the storage of high-risk, look a-like sound a-like, hazardous, controlled substances, corrosive chemical, flammable liquids, and refrigerated items. </li></ul> <p>Ward Stock/Inventory Management</p> <ul> <li>Receives, checks, rotates, and places stock received from pharmacy procurement section and identifies and reconciles any discrepancies to assure correct items have been received. </li><li>Fills automatic replenishment requests for ward stock or clinic stock. </li><li>Inventories and replenishes emergency code cart trays and code carts. </li><li>Maintains, verifies, stocks, and replaces pharmaceutical drug items stored in ADCs (i.e., Pyxis or equivalent). Ensures par levels are maintained and drugs due to expire are pulled in accordance with pharmacy procedures. </li></ul> <p>Sterile Product Compounding (as needed)</p> <ul> <li>Responsible for preparation of complicated parenteral admixtures to be delivered in the form of large volume, small volume, infusion syringe, etc. and for the manufacturing of parenteral nutritional admixtures under a sterile environment. </li><li>Reviews the medication order request, selecting and measuring appropriate ingredients, mixing ingredients in a safe and effective manner, inspecting the final product for accuracy, suitability, and elegance, labeling the product, maintaining records of the manufacturer process and supplying the product for delivery only after a final check has been performed by pharmacist. </li><li>Appropriately and effectively uses IV room equipment (i.e., repeater pump) and technology (i.e., IV Prep). </li><li>Use standard protocols to determine accurate expiration date for each batch prepared. </li><li>When compounding complex admixtures (parenteral nutrition) manufacturing, steps with ingredients are set up for clarification and approval by the pharmacist prior to completing each step in the manufacturing process. </li></ul> <p>Controlled Substance Activities (as needed)</p> <ul> <li>Maintains accurate electronic inventory through decrementing prescriptions, receiving inventory, stocking and transferring to ADCs, dispensing on green sheets, and transferring to other dispensing vaults. </li><li>Ensures that proper handoff occurs when receiving or delivering controlled substances, always assuring that these medications are never left unsecured. </li><li>Ensures all controlled substance procedures abide by all laws, regulations, directives, policies, and procedures applying to the dispensing, storage, transfer, inventory, destruction, and accountability of controlled substances. </li><li>Assists controlled substance inspectors with inventory and accountability inspections. </li><li>Ability to prepare, fill (including prepacking), and dispense controlled substance according to policies and procedures. </li><li>Prepares, operates, restocks, and maintains complex ADCs (i.e., Pyxis) and all necessary equipment supporting pharmaceutical patient care functions. </li><li>Resolves inventory discrepancies immediately and reports discrepancies to pharmacy supervisor. </li><li>Maintains security of controlled substances. </li></ul> <p>Other</p> <ul> <li>Pharmacy Technician will perform all other related duties as assigned by pharmacy supervisor or designee. </li></ul> <p>Work Schedule: Monday - Friday 0730-1600, rotating evenings, weekend & holidays</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #: 11654-F</p> <p>Permanent Change of Station (PCS): Not Authorized</p> <p>Help</p> <p>Requirements</p> <p>Conditions of Employment</p> <ul> <li>You must be a U.S. Citizen to apply for this job. </li><li>All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. It is a requirement that all HCP to receive annual seasonal influenza vaccination or obtain an exemption for medical or religious reasons. Wearing a face mask is required when an exemption to the influenza vaccination has been granted. HCP in violation of this directive may face disciplinary action up to and including removal from federal service. HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased medical facilities. HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities. HPTs may be paid or unpaid and include residents, interns, fellows and students.
