As a Registered Nurse (RN) in Home Health, you'll play a vital role in helping patients navigate their health journey while enjoying the autonomy, flexibility, and purpose that comes with caring for patients one-on-one.</p> <p>Why Join Elara Caring?</p> <ul> <li> <p>Flexible scheduling that supports work-life balance</p> </li><li> <p>Competitive compensation + mileage reimbursement</p> </li><li> <p>Paid orientation and ongoing training</p> </li><li> <p>Supportive, collaborative team environment</p> </li><li> <p>The opportunity to make a real impact-every single day</p> </li></ul> <p>As a Home Health RN, you will:</p> <ul> <li> <p>Deliver skilled nursing care to patients in their homes in accordance with physician orders and the individualized plan of care</p> </li><li> <p>Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations</p> </li><li> <p>Develop, implement, and update personalized care plans based on patient needs and progress</p> </li><li> <p>Provide education and support to patients, families, and caregivers to promote independence and improve outcomes</p> </li><li> <p>Monitor patient conditions and communicate changes to physicians, Clinical Supervisors, and care team members</p> </li><li> <p>Coordinate care with an interdisciplinary team to ensure continuity and quality of care</p> </li><li> <p>Accurately document patient visits, assessments, and care provided in accordance with company standards and regulatory requirements</p> </li><li> <p>Ensure compliance with HIPAA and maintain strict patient confidentiality</p> </li><li> <p>Promote a safe environment by identifying and addressing potential risks in the home setting</p> </li></ul> <p>What You'll Need</p> <ul> <li> <p>Current, unrestricted Registered Nurse (RN) license in the state of practice</p> </li><li> <p>Minimum of 1 year of clinical nursing experience (home health experience preferred)</p> </li><li> <p>Strong assessment, critical thinking, and communication skills</p> </li><li> <p>Ability to work independently and manage a patient caseload</p> </li><li> <p>Willingness and ability to travel within assigned territory (50%)</p> </li><li> <p>Valid driver's license, reliable transportation, and current auto insurance</p> </li><li> <p>Ability to lift/move up to 50 - 100 lbs and assist with patient mobility</p> </li><li> <p>Associate or bachelor's degree in nursing (BSN preferred)</p> </li><li> <p>Experience with HomeCare HomeBase (preferred)</p> </li></ul> <p>Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
