For more information on UofL Health, go to www.UofLHealth.org.</p> <p>Our Mission</p> <p>As an academic health care system, we will transform the health of the communities we serve through compassionate, innovative, patient-centered care</p> <p>Responsibilities</p> <p>The Community Health Worker (Trauma Center) implements the hospital-based violence intervention program as a standard of care, providing resources and support to patients injured due to interpersonal violence. This program is an initiative of the Trauma Institute's Injury Prevention Program and meets the requirement of the American College of Surgeons for Level I Trauma Center verification.</p> <ul> <li>Identifies patients who have been injured from a violent mechanism (gunshot or stabbing, assault) and connect with patient and/or family within 72 hours of patient presentation to hospital </li><li>Assesses patient needs and offer culturally-competent resources to help patient heal from injury and return safely to the community </li><li>Using active listening, assists patient with setting goal(s) that will help the patient return to pre-injury function and provides follow-up communication to support patient reaching goal(s) </li><li>Communicates regularly with with clinical team on patient needs; serves as a liaison between patient and clinical providers when necessary </li><li>In collaboration with Injury Prevention Manager and Injury Prevention Registrar, assists in maintaining data for tracking, reporting and evaluation purposes </li><li>Identify new and changing community resources for patients </li><li>In collaboration with the hospital Emergency Department and inpatient personnel, identifies and de-escalates potential threatening situations </li><li>Documents all patient interactions in accordance with this position's standard processes </li><li>Attends and participates in departmental and hospital-wide meetingsCoordinates referrals to community partners, when appropriate, including procuring all required consentsEnsures appropriate boundaries maintained with patients and familiesCollects additional data as needed for research, program evaluation, and program funder requirements· Supports trauma program research by suggesting and testing program improvements and collecting data </li><li>Maintains patient and visitor confidentiality </li><li>Maintains compliance with all company policies, procedures and standards of conduct </li><li>Complies with HIPAA privacy and security requirements to maintain confidentiality at all times </li></ul> <p>Performs other duties as assigned</p> <p>Qualifications</p> <p>Education:</p> <ul> <li>High school diploma or GED/ equivalent (required) </li><li>Bachelor's degree or Associate's degree in public health, social work, or other related field (preferred) </li></ul> <p>Experience:</p> <ul> <li>Two to three (2-3) years of experience working with directly people of various ages, backgrounds, ethnicities and life experiences (required) </li><li>Hospital experience (preferred) </li></ul> <p>Case management experience (preferred)</p> <p>#LI-DNI</p>
