<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.</p> <p>The Social Worker position is located within the Community Support Services Program, located in the Social Work Service Line at the James J. Peters VA Medical Center. The Social Worker provides case management and interventions to support veterans who are homeless or at risk for homelessness and are seeking employment services.</p> <p>CWT is a vocational psychosocial rehabilitation program requiring knowledge, insight, and skills for evaluating and placing individuals with disabilities in supported, therapeutic and/or competitive employment. The incumbent independently assesses, identifies, and uses interventions to reduce barriers to employment and to provide referrals to additional resources to aide in removal of barriers to competitive employment.</p> <ul> <li>Independently provides case management services and is responsible for completing biopsychosocial assessments, developing, and implementing treatment plans and aftercare plans, and other direct care such as education to Veterans and their family member(s)/significant other(s) on available resources in the VA and the community. </li><li>Provides appropriate clinical documentation for all contacts with or about Veterans in a complete, confidential, professional manner. </li><li>Utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with the Veteran's ability to achieve employment. </li><li>Works with the Veteran to realize the Veteran's treatment goals and reassesses and revises goals as indicated by the Veteran's engagement in treatment. </li><li>Collects and submits Veteran information as outlined by NEPEC/HOMES evaluations procedures. </li><li>Provides outreach services for CWT in the community, including but not limited to shelters, mobile unit events, health fairs, potential employers. </li><li>Effectively communicates with and utilizes other disciplines (e.g., nursing, medicine) to facilitate treatment planning and implementation. With few exceptions, participates effectively in team meetings, treatment planning conferences, etc. Effectively communicates with and utilizes community agencies to facilitate treatment, discharge planning, and continuity of care. With few exceptions, gives evidence of having regular contact and interaction with a variety of community agencies and resources. Collaborates with a variety of community agencies and engages in problem resolution activities. </li><li>Collaborates with diverse multidisciplinary team members in a manner that enhances coordination of comprehensive patient care. The employee's relationship with supervisors, co-workers, patients, visitors, and the public is consistently courteous and cooperative in nature, and overall contributes to the effective operation of the case management program. </li><li>Services will also be provided to formerly homeless, at risk of homelessness and homeless veterans. </li><li>Engages in coordination with the VA and community agencies, organizations, and groups as to learn of current services, resources and/or benefits available for homeless Veterans. </li><li>Actively participates in program planning and development, advising the CWT Supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. </li><li>Actively participates in conference calls and participates in other relevant conference calls and/or meetings as assigned. </li><li>Actively participates in the planning, organization, and surveying of the program for Joint Commission Accreditation and other applicable accreditations. </li></ul> <p>Work Schedule: M-F, 8am - 4:30pm</p> <p>Recruitment Incentive (Sign-on Bonus): Not authorized</p> <p>Permanent Change of Station (Relocation Assistance): Not authorized</p> <p>EDRP Authorized: Former EDRP participants ineligible to apply for incentive.
