<p>Position: Director, Client Partnerships</p> <p>The Director, Client Performance is accountable for oversight of operations and performance to support current clients. The ideal candidate is detail oriented, a strong team player, and has excellent prioritization and time management skills. Main responsibilities include</p> <p>1) Manage contractual requirements to ensure deliverables and reporting are timely and accurate</p> <p>2) Partner with Implementation Team for new client implementations</p> <p>3) Collaborate across the organization to ensure all deliverables and milestones are met to exceed contractual and client expectations.</p> <p>Roles and Responsibilities</p> <ul> <li> <p>Track and review deliverables and reports to ensure timely and quality submission to clients.</p> </li><li> <p>Develop project plan and milestones for new partnerships or program implementations.</p> </li><li> <p>Serve as Project Manager for enhancements and new initiatives for dedicated clients.</p> </li><li> <p>Review Key Performance Indicators and Dashboards for opportunities to improve operational performance.</p> </li><li> <p>Document and maintain current client IT processes and business requirements.</p> </li><li> <p>Function as a key contact and facilitator for defined clients to ensure Monogram is exceeding expectations.</p> </li><li> <p>Ensure CRM is updated and accurate.</p> </li><li> <p>Utilize effective problem-solving skills to overcome both internal and external IT challenges.</p> </li><li> <p>Partner internally and externally to develop operational workflows to improve clinical performance.</p> </li><li> <p>Internal coordination (Clinical Performance, Market Leadership, IT/Medical Economics) to ensure systems and processes are established to meet specific requirements.</p> </li><li> <p>Other duties as assigned.</p> </li></ul> <p>Position Requirements</p> <ul> <li> <p>Phoenix, AZ or Nashville, TN based</p> </li><li> <p>Bachelor's degree from a four-year college</p> </li><li> <p>5+ years' experience with program management, project management, or similar experience</p> </li><li> <p>5+ years' experience working for a dialysis organization, value-based provider group, health plan, and/or vendor contracting with health plans delegated for services</p> </li><li> <p>Detail oriented, excellent prioritization, and time management skills</p> </li><li> <p>Expertise partnering with internal departments to drive collaboration and results</p> </li><li> <p>Highly proficient in Microsoft Excel, Visio, Word, and PowerPoint</p> </li><li> <p>Experience working in high-growth environment preferred</p> </li><li> <p>Valid driver's license</p> </li></ul> <p>Benefits</p> <ul> <li> <p>Opportunity to work in a dynamic, fast-paced and innovative value-based provider organization that is transforming the delivery of kidney care</p> </li><li> <p>Competitive salary and opportunity to participate in the company's bonus program</p> </li><li> <p>Comprehensive medical, dental, vision and life insurance</p> </li><li> <p>Flexible paid leave and vacation policy</p> </li><li> <p>401(k) plan with matching contributions</p> </li></ul> <p>About Monogram Health</p> <p>Monogram Health is a leading multispecialty provider of in-home, evidence-based care for the most complex of patients who have multiple chronic conditions.
