We are looking for talented and highly-motivated individuals who demonstrate a natural desire to support the meaningful work of community oncologists and the patients we serve.</p> <p>Job Description:</p> <p>The Psychosocial Oncology Coordinator will facilitate screening and triage of all patients referred for Psychosocial Oncology services. The coordinator will explain the services offered by the Psychosocial Oncology team, provide psychoeducation, assess patients' level of need for mental health support, determine appropriate external and internal referrals, coordinate appointment scheduling based on the appropriate level of care, and facilitate patients' use of telemedicine.</p> <p>ESSENTIAL FUNCTIONS:</p> <ul> <li>Educate clinical providers, patients, and families about Psychosocial Oncology and connect them to the Tennessee Oncology Psychosocial Oncology service through internal referrals. </li><li>Facilitate initial contact and screen patients with cancer and their caregivers who are referred to the Psychosocial Oncology team to determine: (1) appropriateness of referral, (2) urgency of referral, and (3) readiness for referral. </li><li>Connect patients with appropriate resources and services. </li><li>Coordinate scheduling of Psychosocial Oncology patient appointments. Schedule telemedicine initial appointments across clinics and clinicians. Confirm appointments prior to the day of service. </li><li>Educate and assist patients regarding use of telemedicine platform. </li><li>Manage paperwork including collection of consent forms. </li><li>Maintain accurate and timely documentation of patient contacts. </li><li>Participate in team meetings. </li></ul> <p>KNOWLEDGE, SKILLS & ABILITIES:</p> <ul> <li>Ability to understand, relate to, and communicate with patients with serious illnesses and their families/caregivers </li><li>In-depth knowledge of Psychosocial Oncology services, internal supportive services, and community resources available to patients and families </li><li>Ability to use counseling skills, problem-solving techniques, and brief crisis intervention skills to support patients being assessed for Psychosocial Oncology services via telephone or teleconferencing </li><li>Ability to communicate effectively with all levels of clinical staff to coordinate care for a patient (front office schedulers, medical assistants, nurses, advanced practice providers, physicians) </li><li>Ability to plan, organize, and complete assigned tasks </li><li>Ability to work independently and as part of a team </li><li>Ability to maintain the highest level of confidentiality and privacy regarding patient contacts </li><li>Strong interpersonal skills </li><li>Proficiency in the use of computers and clinical computer software including electronic medical records, and Microsoft Word, PowerPoint, and Excel. </li></ul> <p>EDUCATION & EXPERIENCE:</p> <ul> <li>BS in psychology, social work, or related field required, Masters level preferred </li><li>Minimum 2 years' experience in medical or related field (e.g., human services, psychiatry, psychology) </li><li>Experience with scheduling and triage preferred </li><li>Comfortable with computers, fax, telephone, telemedicine, MS Office (including MS Outlook, Word, Excel and PowerPoint) </li><li>Excellent time management and organizational skills essential </li></ul> <p>PHYSICAL REQUIREMENTS:</p> <p>Job Title: Psychosocial Oncology Coordinator</p> <p>Physical Activity Required</p> <p>Amount of time</p> <p>None</p> <p>Less than 1/3</p> <p>(Occasionally)</p> <p>1/3 to 2/3</p> <p>(Frequently)</p> <p>More than 2/3</p> <p>(Regularly)</p> <p>Standing</p> <p>x</p> <p>Walking</p> <p>x</p> <p>Sitting</p> <p>x</p> <p>Fingering or manual dexterity</p> <p>x</p> <p>Repetitive finger motion</p> <p>x</p> <p>Lifting or exerting force</p> <p>Up to 10 pounds</p> <p>x</p> <p>Up to 25 pounds</p> <p>Up to 50 pounds</p> <p>Up to 100 pounds</p> <p>Over 100 pounds</p> <p>Reaching or stretching</p> <p>x</p> <p>Climbing or balancing</p> <p>x</p> <p>Crouching or stooping</p> <p>x</p> <p>Speaking</p> <p>x</p> <p>Hearing</p> <p>x</p> <p>Seeing (with correction)</p> <p>x</p>
