With three (3) conveniently located health centers, three (3) behavioral health sites and a friendly welcoming staff, PHC aspires to serve all the health care and wellness needs of the entire family.</p> <p>About the Role:</p> <p>The Outreach Nurse - Mobile Health & Community Engagement is responsible for delivering clinical care, preventive screenings, and health education in community-based settings to improve access to care for underserved populations. This role integrates nursing practice, population health, and patient navigation to support care gap closure, chronic disease management, and preventive health initiatives.</p> <p>The Outreach Nurse ensures that all outreach services and education align with organizational capabilities and are based on identified community health needs in accordance with AAAHC and HRSA standards. This position plays a key role in connecting community members to primary care services and converting outreach encounters into scheduled and completed clinical visits.</p> <p>Duties and Responsibilities:</p> <ul> <li> <p>Provide clinical services in community and mobile health settings, including vital signs, screenings, vaccinations, and basic triage within scope of practice</p> </li><li> <p>Deliver culturally competent, evidence-based health education related to chronic disease prevention, women's health, immunizations, and wellness</p> </li><li> <p>Ensure all health education and outreach messaging accurately reflects services provided and aligns with community needs assessments (AAAHC compliance)</p> </li><li> <p>Assist patients with scheduling primary care and specialty appointments and establishing a Primary Care Provider (PCP)</p> </li><li> <p>Conduct follow-up outreach to support appointment adherence, care gap closure, and continuity of care</p> </li><li> <p>Identify and support patients eligible for preventive screenings (e.g., breast, cervical, colorectal) and chronic disease management services</p> </li><li> <p>Coordinate care with clinical teams, patient access, and social services to address medical and social needs</p> </li><li> <p>Utilize EHR and population health tools to document encounters, track outcomes, and support quality metrics</p> </li><li> <p>Participate in community outreach events including health fairs, schools, faith-based organizations, and community centers</p> </li><li> <p>Support mobile health unit operations, including setup, supply management, and maintaining clinical readiness</p> </li><li> <p>Ensure compliance with infection control, documentation standards, and organizational policies</p> </li><li> <p>Track outreach outcomes including patient engagement, services provided, and conversion to scheduled and completed visits</p> </li></ul> <p>Requirements and Skills:</p> <ul> <li> <p>Strong clinical assessment and nursing skills in community and outpatient settings</p> </li><li> <p>Knowledge of preventive care, chronic disease management, and population health strategies</p> </li><li> <p>Ability to provide effective patient education and engage diverse populations</p> </li><li> <p>Strong communication, interpersonal, and care coordination skills</p> </li><li> <p>Proficiency in Electronic Health Record (EHR) systems (e.g., NextGen, EPIC)</p> </li><li> <p>Ability to work independently in community settings and collaboratively with multidisciplinary teams</p> </li><li> <p>Strong organizational skills with attention to detail and documentation accuracy</p> </li><li> <p>Cultural competency and ability to build trust within underserved communities</p> </li></ul> <p>Minimum Qualifications:</p> <ul> <li> <p>Current and valid Licensed Practical Nurse (LPN) license in the state of Missouri</p> </li><li> <p>Completion of an accredited practical nursing program</p> </li><li> <p>Basic Life Support (BLS) certification</p> </li></ul> <p>Preferred Qualifications:</p> <ul> <li> <p>Experience working in a community health or outpatient care setting</p> </li><li> <p>Additional certifications such as IV therapy or wound care</p> </li><li> <p>Familiarity with electronic health record (EHR) systems</p> </li><li> <p>Strong organizational and time management skills</p> </li><li> <p>Bilingual abilities to support diverse patient populations</p> </li></ul>
