<p>Job Title: Wound Care Nurse</p> <p>Job Description</p> <p>We are seeking a dedicated Wound Care Nurse to join our team, providing expert care and management of various types of wounds in a home health setting. The ideal candidate will have a strong skill in wound care, excellent communication skills, and the ability to work collaboratively with an interdisciplinary team.</p> <p>Responsibilities</p> <ul> <li>Conduct comprehensive assessments of wounds including pressure injuries, surgical wounds, diabetic ulcers, venous/arterial ulcers, and other skin breakdowns. </li><li>Develop and implement individualized wound care treatment plans in collaboration with the patient's physician. </li><li>Provide direct wound care, such as debridement, packing, and dressing changes, using advanced wound products. </li><li>Monitor wound healing progression and document in the Homecare HomeBase (HCHB). </li><li>Educate patients and caregivers on wound care, infection prevention, nutrition, and self-care techniques. </li><li>Collaborate with the interdisciplinary team, including physical therapists, occupational therapists, and case managers. </li><li>Communicate changes in patient condition or treatment needs promptly to the physician and care team. </li><li>Maintain accurate, timely, and detailed clinical documentation. </li><li>Follow infection control procedures and universal precautions during all care activities. </li><li>Perform OASIS assessments, Start of Care (SOC), Resumption of Care (ROC), and other assessments as needed. </li></ul> <p>Essential Skills</p> <ul> <li>Active Connecticut RN License. </li><li>Minimum 1 year of wound care experience in home health. </li><li>Wound care certification (WCC, CWOCN, or equivalent) strongly preferred. </li><li>Familiarity with home health regulations, OASIS documentation, and HCHB. </li></ul> <p>Additional Skills & Qualifications</p> <ul> <li>Time management and autonomy: Ability to manage a caseload independently and prioritize patient visits efficiently. </li><li>Patient education and advocacy: Skill in educating patients and families about care plans, medications, and recovery strategies. </li><li>Technology proficiency: Comfort with mobile documentation tools, EMR systems, and telehealth platforms. </li><li>Cultural competency: Ability to provide respectful and effective care across diverse populations. </li><li>Crisis management and adaptability: Responding calmly and effectively to urgent changes in patient condition or environment. </li><li>BSN Degree (Preferred but not required). </li><li>Experience with chronic disease management. </li><li>Certification in wound care or IV therapy. </li><li>HIPAA compliance knowledge. </li></ul> <p>Work Environment</p> <p>Work hours are from 8:30 AM to 5:00 PM, with travel within the surrounding towns of Greenwich in Fairfield County.
