<p>Job Title: SCO Geriatric Support Service Coordinator</p> <p>Business Unit: Senior Care Options (SCO)</p> <p>Department: SCO</p> <p>Reports to: Program Manager</p> <p>FLSA: Non-exempt</p> <p>Classification: Regular Full-Time (35 hours)</p> <p>Grade: B - Union Position</p> <p>Salary Range: $47,000</p> <p>SUMMARY OVERVIEW</p> <p>Under the general direction of the Program Manager, but working collaboratively with the Senior Care Options (SCO) representative(s), the SCO Case Manager plays an integral part of the case management functioning of the agency. The SCO Case manager will provide initial assessment and ongoing assessments of the SCO client's needs including eligibility for any relevant Boston Senior Home Care (BSHC) service and other community resources. Furthermore, the SCO Case Manager will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment. This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation. The SCO Case Manager is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, SCO representatives, vendors, potential referral sources and other community agencies. SCO Case Managers actively participate in BSHC organizational life.</p> <p>ESSENTIAL FUNCTIONS</p> <p>Essential Job functions: Duties <strong><em>1____ through </em></strong>9____are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.</p> <ul> <li>Through home visits conduct multi-dimensional assessment of elders' functional and social needs incorporating eligibility/need for services, special programs and other community resources. </li><li>Provide ongoing assessment of client needs/status through contact with collaterals, informal supports and contact with the client via phone and regularly scheduled home visits. Adjust service planning and follow-up as indicated. </li><li>Through a needs assessment, knowledge of resources and problem solving skills develop a service plan that incorporates the areas identified within the assessment. Seek supervision for ongoing case management direction, high-risk client issues, vendor needs/issues, etc. </li><li>Initiate and follow up with an interdisciplinary consultation and review with SCO personnel for SCO GSSCs service planning recommendations and obtain approval of those services. Implement the plan utilizing all available resources; BSHC vendors/programs and other community resources. </li><li>Monitor the efficacy and quality of services provided by BSHC through contacts with client, their family, collateral contacts and vendor monitoring. </li><li>Maintain the client record through timely documentation that is complete and accurate of all related communications and required forms. </li><li>Provide ongoing advocacy for client, as follows: </li><li>Exhibit working knowledge of BSHC services/programs </li><li>Exhibit working knowledge of other community programs </li><li>Continually assist in the development of community resources knowledge within BSHC </li><li>Seek peer or supervisory consultation to explore unfamiliar resource areas. </li><li>Contribute to organizational life through active participation in team activities, committees, and work groups. Promote a positive working environment through cooperative and respectful interaction with others. </li><li>Through all external contacts with vendors, collaterals, and community at large continually provide outreach efforts as a BSHC representative in order to ensure access to services for all client groups. </li></ul> <p>COMPETENCIES</p> <ul> <li>Sensitivity to elder and diversity issues. </li><li>Commitment to maintaining clients at home with dignity. </li><li>Ability to effectively communicate in person, in writing, and by telephone. </li><li>Ability to effectively utilize BSHC's computer and information systems. </li></ul> <p>SUPERVISORY RESPONSIBILITY</p> <p>This position has no supervisory responsibilities.</p> <p>WORK ENVIRONMENT</p> <p>This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, copiers, filing cabinets.</p> <p>PHYSICAL DEMANDS</p> <p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.</p> <ul> <li>The employee must be able to carry 15 to 20 lbs. </li><li>The employee is regularly required to talk and hear </li><li>The employee is frequently is required to stand, walk, use hands to finger, handle or feel, and reach with hands and arms </li></ul> <p>POSITION TYPE/EXPECTED HOURS OF WORK</p> <p>This is a full-time position, Monday-Friday, 9:00 AM - 5:00 PM.</p> <p>TRAVEL</p> <p>This position requires up to 70% travel.
