We learn, grow, and achieve more - in our careers and in our communities.</p> <p>Job Description Summary:</p> <p>Community Health Worker (CHW) will work with patients at OhioHealth who are identified as having non-clinical barriers to engaging in treatment plans and recommendations. The goal of the CHW is to work with the multidisciplinary team to identify barriers to the patient's health care and coordinate support services and community based resources to address those barriers with the patient. In addition, this position provides individual consultation and follow up based on Social Determinants of Health, patient demographics and lack of resources, and coordinates support for clients.</p> <p>Responsibilities And Duties:</p> <p>Direct patient support (60%):</p> <p>Must have access to reliable transportation</p> <p>Meet individually with patients (face to face or telephonic) to conduct intake interviews and identify non-clinical barriers</p> <p>May include home visits or community based visits based on severity of patient risk.</p> <p>Coordinate support services or community resources for patients and provide basic health promotion education.</p> <p>Facilitates patients adherence to treatment plans and help access affordableMedication</p> <p>Develop and track measurable and time bound goals with patient</p> <p>Maintain HIPPA compliance</p> <p>Communicate importance of adherence to plan of care (developed by multidisciplinary team)</p> <p>Convey importance of healthy lifestyle choices (nutrition, exercise, stressmgmt.) and adverse health impacts of smoking, drinking, and drug use</p> <p>Communication with health care team (20%):</p> <p>Document interactions with patients and communicate regularly with Care team; Develop and maintain relationships with community resources to ensure Coordination of care for patients</p> <p>Engage in multidisciplinary care team huddles</p> <p>Maintain positive relationship between OhioHealth and community resources</p> <p>Report how patients demeanor may impact treatment (crying, angry, etc)</p> <p>Document important information ancillary to medical treatment (stressors, children, domestic violence, involvement of partner, etc)</p> <p>Recognize and report signs of family violence, abuse, neglect, etc.</p> <p>Administrative tasks (20%):</p> <p>Maintain accurate and timely patient records.</p> <p>Serve as community liaison between OhioHealth and local community agencies.</p> <p>Answers phones/review messages, triages calls to determine needs and appropriate course of action</p> <p>Makes, facilitates, and tracks appropriate referrals</p> <p>Recognize differences in client populations and implications for identifyingAppropriate services</p> <p>Minimum Qualifications:</p> <p>High School or GED (Required)</p> <p>Additional Job Description:</p> <p>This is a temporary position that is not eligible for benefits.
