In this position you will bring trust, comfort, and empathy to all resident interactions and make a positive difference in the lives of our residents.</p> <p>Job Specific Competencies</p> <p>Principal Responsibilities:</p> <p>Core Competencies:</p> <ul> <li> <p>Supports the Trinity/SPHP Vision, Mission, Values in all encounters with residents and families</p> </li><li> <p>Adheres to the SPHP Code of Conduct</p> </li><li> <p>Demonstrates accountability as an individual and team member in meeting basic resident and family needs</p> </li></ul> <p>Clinical Competencies:</p> <p>LPNs provide direct and indirect care under the direction of an RN including procedures, treatments, and related duties as assigned and within the NYS Scope of Practice for an LPN.</p> <ul> <li> <p>Resident support - Deliver compassionate, person-centered care while fostering a sense of dignity and independence for each resident.</p> </li><li> <p>Medication administration - Conduct daily medication pass with accuracy and care in accordance with physician orders and facility guidelines.</p> </li><li> <p>Shift leadership - provide leadership for the care team during your shift, including guidance and support.</p> </li><li> <p>Team collaboration - work closely with resident assistants, other nurses, and management in a cooperative, respectful environment.</p> </li><li> <p>Documentation - complete required documentation promptly and accurately to support resident care.</p> </li><li> <p>Consult with the RN on any changes in a care plan or schedule and communicate with patients and their families about their care plan.</p> </li><li> <p>Attends and actively participates in in-servicing, orienting and educational activities.</p> </li></ul> <p>Education Requirements:</p> <ul> <li> <p>Must be a high school graduate.</p> </li><li> <p>Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).</p> </li><li> <p>Must be licensed with current registration in New York.</p> </li></ul> <p>Must be able to:</p> <ul> <li> <p>Stand, walk, and lift for long periods of time</p> </li><li> <p>Read and write using medical terminology</p> </li><li> <p>Working knowledge of computers including electronic medication administration records, processing of orders, checking email, and preforming required inservicing, along with use of other required applications</p> </li><li> <p>Have the visual and auditory acuity adequate to perform observations and patient care</p> </li><li> <p>Maintain composure and positivity in a high stress environment</p> </li><li> <p>Promote physical, cognitive, and psychosocial well-being of each resident</p> </li><li> <p>Work with residents in a highly personal manner during stages of acute and chronic illness</p> </li></ul> <p>Pay Range: $24.00 - $30.10</p> <p>Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
