<p>Job Summary</p> <p>Facilitates cost-effective, quality patient transitions of care and outcomes by collaborating with staff and providing guidance, education, and support to assigned patients.</p> <p>Job Responsibilities</p> <ul> <li>Responsible for the coordination and facilitation of the plan of care. </li><li>Assessment of complete chart and knowledge of patient's medical condition, financial status, emotional needs, and support systems. </li><li>Collaborates with the Medical Director, Director, and Manager of Case Management on complex discharge transitions and delays of service. </li><li>Interviews patients, assesses priorities and needs to develop a safe discharge plan. </li><li>Collaborates with the interdisciplinary team to ensure patient quality of care and safety of discharge. </li><li>Works in collaboration with the social worker and Interdisciplinary team to ensure quality support of care and safety of discharge. </li><li>Keeps plans up to date as patient progresses toward discharge. </li><li>Provides timely and concise documentation in the medical record, the assessment, concerns, needs and discharge plan. </li><li>Assists patients/families with referrals to community agencies and provides education and resource information. </li><li>Provides education to physicians, staff, and patients/families. </li><li>Supervises and/or trains other Case Managers, students or interns as needed. </li></ul> <p>Qualifications</p> <ul> <li>Bachelor's degree or higher in Nursing required upon hire </li><li>No experience required </li><li>RN - Licensed as a Registered Nurse.
