<p>Purpose:</p> <p>Assists families in connecting with community-based healthcare, human services, and community resources through Informing, Care Coordination, Presumptive Eligibility, outreach and referral activities.</p> <p>Responsibilities:</p> <ul> <li>Collaboratively develops and maintains accurate and thorough resource directories and databases of community-based family and child health organizations, programs, services and experts. </li><li>Conducts outreach efforts to ensure information about program benefits, services and resources is marketed to appropriate audiences. </li><li>Contacts clients by phone, email, and/or through home or clinic visits to assess needs and successfully link families to health insurance access and medical/dental homes; assists families with scheduling appointments, finding interpretation services, childcare, transportation and other resources as necessary. </li><li>Educates families about the EPSDT Care for Kids Program and the importance of preventive health and dental care for children. </li><li>Provides health education as needed, on topics such as: immunization schedules, breastfeeding and formula feeding, pre/post-natal care, oral health care and pregnancy, childhood growth and development, SIDS, car seat safety, maternal depression, and common childhood illnesses. </li><li>Uses a family-centered approach in advocating for, educating and supporting families as they select and use effective resources to meet their needs. </li><li>Collaborates with providers and community agencies who are involved with the family. </li><li>Provides follow-up education for families and monitors health and dental plans and services. </li><li>Uses a family-centered approach in providing support and assistance as families become independent health care consumers. </li><li>Performs accurate and timely data entry and documentation. </li><li>Enrolls pregnant women and children for Medicaid Presumptive Eligibility using the MPEP portal. </li><li>Distributes outreach materials to community partners and/or interacts with the public at outreach events in an attempt to link families to health insurance and medical and dental homes. </li></ul> <p>Qualifications:</p> <ul> <li>Family Outreach Specialist: Associate degree in social work, sociology, psychology, health education or nursing </li><li>At least 1 year of experience providing home visitation, care coordination or resource and referral services preferred </li><li>Experience working with diverse communities preferred </li><li>Ability to communicate effectively, both verbally and in written English; bilingual in Spanish preferred </li><li>Aptitude for technical writing </li><li>Ability to identify family strengths and needs for family-centered support </li><li>Knowledge of community resources </li><li>Analytical skills to interpret information </li><li>Ability to work independently and exercise sound decision-making </li><li>Strong keyboarding and computer skills with the ability to learn work-related software; knowledge of Microsoft Office, including Word, Excel and Outlook; knowledge of database use and management </li><li>Time management skills with the ability to effectively plan, organize and prioritize work </li><li>Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire) </li><li>Must have valid driver's license, auto liability insurance and reliable transportation if driving is required </li><li>Successful completion of pre-employment physical, drug screen, 2-step TB test and background check </li></ul> <p>Working Conditions:</p> <ul> <li>Physical requirements: Light work, sitting most of the time, exerting up to 20 pounds of force frequently, and/or a small amount of force constantly to move objects. </li><li>Physical activities: Kneeling, Reaching, Pushing, Talking, Hearing, Repetitive motions, Standing, Stooping, Walking, Lifting, Fingering (picking, pinching, typing, etc.), Grasping (applying pressure with fingers and palm), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations) </li><li>Environment: Extreme temperatures, Hazards (chemicals, infectious diseases), Atmospheric conditions (fumes, odors, dusts, mists, gases) </li><li>Time: Ability to be flexible with a varying schedule and programmatic requirements </li><li>Travel: Required between locations </li></ul> <p>These characteristics are representative of those an employee encounters while performing the essential functions of this job.
