<p>RN Stroke Program # Joint Replacement Coordinator Boulder Community Health, Boulder, CO 80303 $38.45#- $57.68#an hour - Full-Time Days, 30 hours per week.# +$2.50/hr for weekend shifts.# Boulder Community Health is seeking a registered professional nurse who demonstrates expertise in providing advanced nursing process skills to the stroke patient population throughout the continuum of care. Certified NIH Stroke Scale within 90-day conditional period required. ## #b. Certified Stroke Registered Nurse (CSRN) required within 18 months. ## #c. Membership- American Association of Neuroscience Nurses (AANN) required within 12 months. ## #d. Membership- National Association of Orthopedic Nurses (NAON) preferred.# Responsibilities #Coordination with Medical Staff Stroke and Joint Replacement Program Directors for maintenance and growth of the Primary Stroke program, evaluation of outcomes and performance improvement. #Development of multidisciplinary Stroke and Joint Replacement Committees, co-chairing with Stroke and Joint Replacement Medical Directors and development of committee agendas and review of meeting minutes #Develops linkages with the community to increase access to Stroke center resources. #Demonstrates organizational, time-management, and priority-setting skills. #Demonstrates self-direction and flexibility in performance of job responsibilities. #Participates in the design of the internal/external marketing of the programs. #Develops and presents public education programs, for prevention, diagnosis, recognition, and treatment of Stroke. #Supports physician community lectures on Stroke and Joint Replacement in conjunction with Medical Staff office and Public Relations. #Assess and facilitates communication within the Stroke Joint Replacement and programs including other departments that impact the diagnostic process, treatment and care of these patients #Develop and collaborate on patient teaching materials. #Writing, reviewing, and revising Stroke Program and Joint Replacement policies, procedures, protocols, and practice standards. #Initiates and maintains disease-specific Joint Commission certification as Primary Stroke Center. #Staff development regarding care of the Stroke and Joint Replacement patient population, and development of related materials and input on related PI projects. #Initiates, implements and provides follow through with performance improvement projects concerning Stroke and Joint Replacement patients, incorporating data and information to continuously improve care and practice to enhance outcomes. #For Primary Stroke designation, collects and reports performance improvement measures to GWTG, CMS, the Joint Commission and internal Stroke and Patient Safety committees. #In collaboration with the interdisciplinary team develops, maintains, and monitors stroke practice, clinical practice guidelines, order sets and clinical protocols utilizing the American Heart Association, AANN and institute of Neurological Disorders and Stroke (NIH) guidelines. #Exercises independent judgment in the assessment, diagnosis, and initiation of nursing/medical processes and procedure in line with the nurse practice act of Colorado. #Seeks professional development through attending conventions, workshops, skills lab and other professional meetings.
