This role evaluates patients admitted with a diagnosis of chronic lung disease for inclusion criteria to determine the need for disease-specific education and follow-up to reduce hospital admissions and readmissions. This role provides education, materials, and/or referrals appropriate for the patient's educational level and specific needs while maintaining accurate documentation and data for reporting purposes.</p> <p>About This Role:</p> <ul> <li>Full Time </li><li>Day Shift </li><li>Monday - Friday </li><li>Primarily working with COPD patients. </li></ul> <p>Responsibilities:</p> <ul> <li>Responsible for the oversight of resource management, discharge planning, care facilitation, utilization management and pulmonary patient education and related duties by working collaboratively with the interdisciplinary team to achieve desired clinical, service and cost outcomes on the A6N Progressive Care Unit. </li><li>Ensures that patients are assigned the appropriate level of care, receive the appropriate services for the level of care, and are transitioned to the appropriate post-hospital level of care. </li><li>Provide valuable information for consideration in clinical decision making, patient throughput and discharge planning. </li><li>Completes pulmonary patient education on non-PCU patients on A6N. </li></ul> <p>Requirements:</p> <ul> <li>Associate of Science degree from a CoARC accredited Respiratory Care Program. </li><li>NBRC Registered Respiratory Therapist. </li><li>Current state of Indiana license as Respiratory Therapist. </li><li>A minimum of 3-5 years relevant clinical experience in acute care setting, </li><li>Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. </li><li>Demonstrated ability to collaborate and communicate with physicians. </li><li>Demonstrated excellent pulmonary knowledge and ventilator skills. </li><li>Ability to learn/develop resource management skills. </li><li>Ability to operationalize quality improvement measures and to collect and communicate measures of patient care related data. </li><li>Ability to communicate with outside agencies. </li><li>Demonstrated leadership skills and strong work organization skills in order to motivate members of a multidisciplinary team. </li><li>Ability to be flexible in clinical work environment. </li><li>Demonstrated ability to troubleshoot patient and clinical situations. </li><li>Excellent ability to teach patients and families. </li><li>Strong written and verbal communication skills. </li><li>Critical thinking skills. </li><li>Ability to work independently. </li><li>Ability to manage multiple projects simultaneously. </li><li>Knowledge of software applications. </li></ul> <p>Outside of working with groundbreaking technologies and therapies, our Respiratory Team get the opportunity to grow their medical knowledge and skills.
