<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p> <p>The Life Skills Center/Psychosocial Rehabilitation and Recovery Center (PRRC) is an outpatient transitional learning center designed to provide psychosocial rehabilitation and recovery-oriented care and community integration to Veterans living with serious mental illness and severe functional impairment.</p> <ul> <li>The incumbent is assigned to the PRRC and, as a PRRC Social Worker, is responsible for providing individualized psychosocial rehabilitation and recovery-oriented care to Veterans living with serious mental illness and severe functional impairment, and who are eligible for PRRC services. </li><li>Maintain a caseload of Veterans enrolled in PRRC and provide individual therapy/recovery advising, case management, care coordination, group facilitation, crisis intervention, and community integration support as clinically indicated. </li><li>Groups offered by the incumbent will be Recovery-Oriented, curriculum-based based and will involve psychotherapy, skills development, and psychoeducation. </li><li>The incumbent may also be asked to facilitate Recovery- Oriented Bridge groups throughout the hospital and in the community, to educate others about the Recovery Model, and to make eligible Veterans aware of PRRC services. </li><li>Community integration support provided by the incumbent will focus on developing skills for enhanced community integration and supporting opportunities to practice those skills with a goal of improving functional independence within their communities of choice. </li><li>Remain informed of best practices within the fields of mental health and psychosocial rehabilitation and recovery. </li><li>The incumbent may provide best practices and evidence-based treatment depending on training and certification. Examples of practice may include Social Skills Training (SST), Illness Management and Recovery (IMR), Motivational Interviewing, and Cognitive Behavior Therapy (CBT). </li><li>Recognize family members and other natural supports as vital contributors to a Veteran's recovery. Based on the needs/wishes of the Veteran, the incumbent is expected to include these contributors in the Veteran's care, as clinically indicated. </li><li>Act as an advocate on behalf of Veterans to ensure comprehensive service delivery to include actions such as linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Follow VHA Handbooks, local policy and SOPs for the Medical Center and the PRRC Program, and policies and documentation requirements as required for The Joint Commission (TJC) accreditation. </li><li>Performs accountability functions such as ensuring they are appropriately documenting all contacts with patients/families in the medical record in accordance with VA Medical Center policies. </li><li>Complies with criminal conflict of interest laws and Executive Branch Standard of Conduct to avoid conflicts of interest while serving as the PRRC social worker. </li><li>Attends PRRC conference calls and trainings offered by the National PRRC Program as assigned by the supervisor. </li><li>Participates in committee assignments, as deemed necessary by the supervisor. Conducts psychosocial assessments to determine the needs of Veterans and family members. </li><li>Actively participates in the treatment planning process with other disciplines and provides measurement-based care. </li><li>Provides appropriate services based on Veterans' needs, acuity level, and preferences for care. Veterans will be reassessed regularly, no less than quarterly, and in accordance with local policy, for changes in their needs, acuity level, and preferences for care. </li><li>Utilizes psychosocial rehabilitation and recovery practices and other evidence-based practice models to promote Veteran engagement and self-efficacy. </li><li>Acts as a consultant to other team/staff members. Acts as a liaison between VA and community resources, to include marketing and public relations duties as needed. </li><li>Provides case management to Veterans and families, as needed, throughout the continuum of care. </li><li>Coordinates treatment and communicates with the care lines to facilitate continuity of care for Veterans. </li><li>Assists Veterans and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness, and death. </li><li>Documents social work interventions and activities in the Veteran's clinical record utilizing CPRS and ensures appropriate hand-offs when transferring patient care. </li><li>Performs other related duties as assigned by the supervisor. </li></ul> <p>Work Schedule: 7:30-1600</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #: 554-50964A, 554-50965A</p> <p>Relocation/Recruitment Incentives: Not authorized</p> <p>EDRP Authorized: Former EDRP participants ineligible to apply for incentive.Contact vhaedrpprogramsupport@va.gov, the EDRP Coordinator for questions/assistance.
