Monday-Friday 8am-4:30pm</p> <p>Summary:</p> <p>The OB/GYN Care Manager is an integral member of the Hudson Headwaters Women's Health Interdisciplinary Team. This individual will play a pivotal role in enhancing the care and support provided to patients, particularly those navigating high-risk pregnancies, domestic violence situations, and other complex social and/or health related needs. The Care Manager will work closely with healthcare providers and the health center team to connect patients with community resources, ensuring they receive the comprehensive support they need to improve their health outcomes and overall well-being.</p> <p>Essential Duties and Responsibilities:</p> <ul> <li>Provide individualized support to patients, particularly those experiencing high-risk pregnancies or other complex health and social concerns, ensuring they feel heard and supported throughout their care journey. </li><li>Identify and connect patients with relevant community-based resources, such as housing assistance, food programs, financial support, and childcare services. </li><li>Act as a point of contact for patients in crisis, including linking them to crisis intervention services and ensuring timely access to appropriate care. </li><li>Provide responsive and nonjudgmental support to individuals in domestic violence relationships, helping them access safety planning resources, counseling, and legal services as needed. </li><li>Work closely with OB/GYN providers and the health center team to create and implement care plans tailored to each patient's individual needs and circumstances. </li><li>Educate patients about available services, programs, and resources, empowering them to make informed decisions about their care and life circumstances. </li><li>Maintain accurate and timely documentation of all patient interactions, resource referrals, and care plans in accordance with the health center's policies and procedures </li><li>Complete annual care management competencies and any other relevant competencies/trainings as needed. </li><li>Recognize the patient as a contributing member of the clinical team </li><li>Serve on network-wide initiatives that support the mission, vision and core values. </li><li>Provide any other services as directed by HHHN to ensure proper care and treatment to the patient and/or families. </li></ul> <p>Qualifications:</p> <p>The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:</p> <ul> <li>Bachelor's Degree in a health or human service field, applied professional experience in case (care) management and/or social work required. Will consider an equivalent combination of experience and education </li><li>Current BLS Certification </li><li>Experience/training in Crisis intervention preferred </li><li>Experience working in fields of Health care, behavioral/mental health, substance/alcohol abuse preferred </li><li>Must have a valid driver license and be able to travel throughout the Network </li><li>Must have strong verbal and written communication skills </li><li>Proficient computer competencies including Microsoft applications, electronic medical records and related databases </li><li>Must be well organized and can effectively manage multiple cases and projects </li><li>Must be self-directed, detail-oriented and motivated </li><li>Must be able to work independently as well as collaborate and communicate effectively with colleagues, supervisors, service delivery partners, other health care professionals and co-workers to build and maintain effective dynamic professional team relationships </li></ul> <p>The pay rate for this position is $24.48/hour.</p>
