<p>POSITION TITLE: Case Manager</p> <p>LOCATION: Beckley, WV</p> <p>STATUS: Full Time, Hourly, Non-Exempt</p> <p>PROGRAM: Homeless Veterans Reintegration Program</p> <p>REPORTS TO: Program Manager</p> <p>INTRODUCTION:</p> <p>Volunteers of America Mid-States (VOA) is a non-profit organization spanning four states that creates positive change in the lives of individuals and communities through a ministry of service. The HVRP Case Manager will be responsible for job placement and retention, supportive services and referrals for enrolled veterans, case documentation, and assistance with the preparation of internal and external reporting.</p> <p>WHAT YOU SHOULD HAVE FOR THIS ROLE:</p> <p>This position requires a Bachelor's degree in social work or related field and less than five years of experience in the field; at least five (5) years of experience in the field and no degree; or a Veteran with three (3) years of related work experience. Requires a demonstrated ability to provide a reasonable combination of the following knowledge, abilities and skills: family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers. Must have a commitment to the principles of the project. Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.</p> <p>RESPONSIBILITIES:</p> <ul> <li>Provides outreach to community to increase awareness of HVRP and recruit Veterans to the program. </li><li>Provides case management duties, including: </li></ul> <p>o Performing assessment and developing/monitoring case plans and conducting necessary follow-up activities.</p> <p>o Establishing linkages with appropriate agencies and service providers in the area/community.</p> <p>o Providing referrals and coordinating with VAMCs and other service providers to provide health care services and daily living services.</p> <p>o Decide how resources are allocated to participants on the basis of need</p> <p>o Educate participants on issues, such as supportive services availability and participant rights.</p> <p>o Providing assistance to participants in obtaining VA benefits.</p> <p>o Providing assistance to participants in obtaining and coordinating of public benefits.</p> <p>o Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, childcare, and housing counseling.</p> <p>o Completing required documentation (including progress notes) within 48 hours of contact.</p> <p>o Demonstrating good clinical judgment in decision making regarding participants.</p> <p>o Demonstrating ability to relate to Veterans and their families in a culturally appropriate manner.</p> <p>o Performance Quality Improvement (PQI) duties as assigned by supervision and PQI Committee.</p> <ul> <li>Work in partnership with other HVRP Case Managers and Outreach Workers, in addition to any SSVF program staff; and participate in regular case consultation. </li><li>Advocate for the Veteran, integrating cultural values into their case plan </li><li>Identify systemic barriers and communicates with organizational leadership about these barriers to work collaboratively to find viable solutions. </li><li>Comply with all policies and procedures of the program and the Council on Accreditation. </li></ul> <p>We are an equal opportunity employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity or expression, pregnancy, age, national origin, disability status, genetic information, protected veteran status, or any other characteristic protected by law.</p>
